超声引导钬激光切开内引流术治疗肾盂旁囊肿伴结石的疗效分析

摘　　要：目的探讨超声引导钬激光切开内引流术治疗肾盂旁囊肿伴结石的疗效。方法回顾性分析2013年8月至2019年10月收治的41例肾盂旁囊肿伴结石患者的资料,其中22例(常规组)患者行常规输尿管软镜钬激光切开内引流术,19例(超声组)患者行超声监视软镜钬激光切开内引流术。比较两组手术时间、出血量、手术成功率、碎石成功率、住院时间、并发症及术后囊肿复发情况。结果超声组与常规组相比手术时间较短[(60.8±13.4)vs.(72.5±15.1)min,P<0.05],出血量较少[(6.3±4.1)vs.(11.4±5.8)mL,P<0.05]。两组手术成功率、碎石成功率、住院时间、并发症等差异无统计学意义(P>0.05),随访6~12月,两组囊肿、结石复发率差异无统计学意义(P>0.05)。结论超声引导钬激光切开内引流术与常规钬激光切开内引流术相比,能更迅速准确地寻找囊肿及结石,具备创伤小、恢复快等优点,值得临床推广。Objective To evaluate the efficacy of ultrasound-guided holmium laser ureteroscopic intrapelvic drainage in the treatment of parapelvic cysts with kidney stones.Methods The clinical data of 41 patients with parapelvic cysts and kidney stones treated during Aug.2013 and Oct.2019 were retrospectively analyzed,including 22 treated with holmium laser ureteroscopic intrapelvic drainage(conventional group),and 19 treated with ultrasound-guided holmium laser ureteroscopic intrapelvic drainage(ultrasonic group).The operation time,intraoperative blood loss,success rate of operation,stone-free rate,average hospitalization time,complications and recurrence rate were compared between the two groups.Results Compared with the conventional group,the ultrasonic group had shorter operation time[(60.8±13.4)vs.(72.5±15.1)min,P<0.05]and less blood loss[(6.3±4.1)vs.(11.4±5.8)mL,P<0.05].There were no significant differences in the success rate of operation,stone-free rate,average hospitalization time,complications and recurrence rate between the two groups(P>0.05).During the follow-up of 6-12 months,there were no significant differences in recurrence rates of parapelvic cysts and kidney stones(P>0.05).Conclusion Compared with conventional holmium laser ureteroscopic intrapelvic drainage,ultrasound-guided operation can locate the cysts and stones in a faster and more accurate manner.It is minimal invasive and patients can recover rapidly.
