温肾活血法对高海拔地区慢性心力衰竭心肌功能及炎症因子的影响

摘　　要：目的观察温肾活血法对高海拔地区慢性心力衰竭患者心肌功能及炎症因子的影响。方法选取2018年5月—2020年5月于青海省中医院及甘肃中医药大学附属医院治疗的慢性心力衰竭患者200例,采用随机分组方式,将高低海拔地区患者各随机分为4组,高海拔温肾活血组、高海拔常规治疗组、低海拔温肾活血组和低海拔常规治疗组,每组50例。对高海拔常规治疗组和低海拔常规治疗组患者进行常规抗心衰治疗。高海拔温肾活血组和低海拔温肾活血组使用温肾活血中药治疗。4组患者均持续治疗3个月。比较4组治疗疗效,治疗前后心功能指标,BNP、cTnl水平变化和血清炎症因子。结果高海拔温肾活血组和低海拔温肾活血组治疗总有效率高于高海拔常规治疗组和低海拔常规治疗组(P<0.05);高海拔温肾活血组治疗后心功能指标LVEF、SV水平高于高海拔常规治疗组,LVEDD水平低于高海拔常规治疗组,低海拔温肾活血组治疗后心功能指标LVEF、SV水平高于低海拔常规治疗组,LVEDD水平低于低海拔常规治疗组(P<0.05);高海拔温肾活血组治疗后BNP、cTnl水平低于高海拔常规治疗组,低海拔温肾活血组治疗后BNP、cTnl水平低于低海拔常规治疗组(P<0.05);高海拔温肾活血组治疗后血清炎症指标IL-6、CPR和TNF-α水平低于高海拔常规治疗组,低海拔温肾活血组治疗后血清炎症指标IL-6、CPR和TNF-α水平低于低海拔常规治疗组(P<0.05)。结论温肾活血汤辅助治疗慢性心力衰竭效果显著,可有效的改善患者的心功能指标,降低BNP、cTnl水平和血清炎症因子,提高治疗疗效,对高海拔地区的慢性心力衰竭患者的治疗效果与低海拔地区患者效果相同。Objective To observe the effect of warming kidney and activating blood circulation method on myocardial function and inflammatory factors in patients with chronic heart failure at high altitude.Method A total of 200 patients with chronic heart failure treated in our hospital and Affiliated Hospital of Gansu University of Traditional Chinese Medicine from May 2018 to May 2020 were randomly divided into four groups:high altitude warming kidney and activating blood circulation method group,high altitude routine treatment group,low altitude warming kidney and activating blood circulation method group,low altitude routine treatment group,50 cases in each.Patients in high altitude routine treatment group and low altitude routine treatment group received routine anti-heart failure treatment.The high and low altitude warming kidney and activating blood circulationgroups were treated with warming kidney and activating blood circulationtraditional Chinese medicine.All the four groups were treated for 3 months.The therapeutic effects,cardiac function indexes,levels of BNP,cTnl and serum inflammatory factors were compared among the four groups before and after treatment.Result The total effective rate of high altitude warming kidney and activating blood circulation group and low altitude warming kidney and activating blood circulation group was higher than that of high altitude routine treatment group and low altitude routine treatment group(P<0.05).After treatment,the levels of LVEF and SV in high altitude warming kidney and activating blood circulation group were higher than those in high altitude routine treatment group,and the levels of LVEDD were lower than those in high altitude routine treatment group and low altitude routine treatment group.After treatment,the levels of LVEF and SV in the high altitude warming kidney and activating blood circulation group were higher than those in the low altitude routine treatment group,and the levels of LVEDD were lower than those in the low altitude routine treatment group(P<0.05)
