芪苈强心胶囊对急性前壁心肌梗死左心功能、血清亲环素A及不良心血管事件的影响分析

摘　　要：目的探讨芪苈强心胶囊对急性前壁心肌梗死左心功能、血清亲环素A和不良心血管事件的影响。方法选择2018年4月—2019年4月急性前壁心肌梗死患者90例,随机分两组,对照组采用标准治疗(抗血小板聚集、调脂稳定斑块、ACE-I或ARB、β受体阻滞剂、利尿剂、硝酸酯类药物);观察组在标准治疗基础上加用芪苈强心胶囊3.6 g/d口服,治疗时间为3个月。比较两组患者的临床疗效,测定左心舒张末期容积(Leftventricular end-diastolic volume,LVEDV)、左心收缩末期容积(Left ventricular end-systolic volume,LVESV)、左心射血分数(Left ventricular ejection fraction,LVEF)及血液中脑钠肽(brain natriuretic peptide,BNP)、高敏C反应蛋白(High-sensitivity C-reactive protein,hs-CRP)、肌钙蛋白I(troponin I,cTnI)、亲环素A水平,记录不良心血管事件。结果与治疗前比较,两组患者治疗后LVEDV、LVESV降低,LVEF升高;与对照组比较,观察组患者治疗后LVEDV、LVESV较低,LVEF较高,差异有统计学意义(P<0.05)。与治疗前比较,两组患者治疗后血BNP、Hs-CRP、cTnI、亲环素A降低;与对照组比,观察组患者治疗后血BNP、Hs-CRP、cTnI、亲环素A较低,差异有统计学意义(P<0.05)。对照组不良心血管事件发生率20.00%(9/45)与观察组6.67%(3/45)比较,差异有统计学意义(P<0.05)。结论芪苈强心胶囊有助于改善急性前壁心肌梗死患者的左心功能,降低血清亲环素A、BNP、cTnI、hs-CRP水平,避免不良心血管事件,提高临床疗效,值得推广。Objective To investigate the effects of Qili Qiangxin Capsule on left ventricular function,serum cyclophilin A and adverse cardiovascular events in patients with acute anterior wall myocardial infarction.Methods Ninety patients with acute anterior wall myocardial infarction from April 2018 to April 2019 were randomly divided into two groups.The control group was treated with routine anti-platelet treatment,lipid-lowering medicine,ACEI or ARB,β-blocker,and nitrates.The observation group was additionally treated with with Qili Qiangxin Capsule orally for 3.6 g/day,and two groups were treated for 3 months.Left ventricular end-diastolic volume(LVEDV),left ventricular end-systolic volume(LVESV),left ventricular ejection fraction(LVEF),brain natriuretic peptide(BNP),high-sensitivity C-reactive protein(hs-CRP)were measured.The levels of hs-CRP,troponin I(cTnI)and cyclophilin A were recorded for adverse cardiovascular events.Results Compared with those before treatment,LVEDV and LVESV decreased and LVEF increased in the two groups after treatment.Compared with those of the control group,LVEDV and LVESV in the observation group were lower and LVEF was higher after treatment,and the difference was statistically significant(P<0.05).Compared with those before treatment,the serum levels of BNP,Hs-CRP,cTnI and cyclophilin A in the two groups decreased after treatment,while those in the observation group were lower than those in the control group(P<0.05).There was significant difference in the incidence of adverse cardiovascular events between the control group(20.00%,9/45)and the observation group(6.67%,3/45)(P<0.05).Conclusion Qili Qiangxin Capsule can improve left ventricular function,reduce serum levels of cyclophilin A,BNP,cTnI and hs-CRP in patients with acute anterior wall myocardial infarction,avoid adverse cardiovascular events and improve clinical efficacy.It is worthy of promoting.
