针灸联合正骨推拿治疗单纯性腰椎间盘突出的疗效及对疼痛和下肢感觉障碍的影响

摘　　要：目的探讨针灸联合正骨推拿治疗单纯性腰椎间盘突出(LDH)的疗效及对疼痛和下肢感觉障碍的影响。方法选取2016年2月—2019年5月来医院就诊的127例单纯性LDH患者作为研究对象。按照治疗方法将患者分为观察组(64例)和对照组(63例)。观察两组患者治疗后疗效。观察两组患者治疗后JOA及NRS评分变化情况。检测患者治疗前后血清炎症因子水平。观察两组患者治疗前后下肢功能障碍评分、腰椎活动度及腰椎曲度变化情况。结果观察组患者总有效率为92.18%(59/64),明显高于对照组的79.37%(50/63)(P<0.05)。治疗后,两组患者VAS评分均下降(P<0.05),观察组下降幅度高于对照组(P<0.05)。治疗后两组患者JOA评分均升高(P<0.05),观察组上升幅度高于对照组(P<0.05)。治疗后,两组患者血清NO、IL-6、P物质及MMP-3水平均下降(P<0.05),其中观察组患者下降程度高于对照组(P<0.05)。治疗后,两组患者下肢感觉障碍评分、腰椎活动度及腰椎曲度均升高(P<0.05),其中观察组上升幅度高于对照组(P<0.05)。结论针灸联合正骨推拿治疗单纯性腰椎间盘突出的疗效显著,能明显减轻患者疼痛感,改善患者下肢感觉障碍。Objective To investigate the effect of bone-setting massage combined with acupuncture on simple lumbar disc herniation(LDH)and its influence on pain and sensory disturbance of lower limbs.Methods 127 patients with simple LDH admitted to our hospital from February 2016 to May 2019 were selected as the study subjects.Patients were divided into observation group(n=64)and control group(n=63)according to the treatment method.To observe the curative effect of two groups of patients after treatment.The changes of JOA and NRS scores were observed in two groups after treatment.The levels of serum inflammatory factors were measured before and after treatment.The changes of lower limb dysfunction score,lumbar motion and lumbar curvature were observed before and after treatment.Results The total effective rate was 92.18%(59/64)in the observation group,which was significantly higher than 79.37%(50/63)in the control group(P<0.05).After treatment,the VAS scores of both groups decreased(P<0.05),and the decrease of the observation group was higher than that of the control group(P<0.05).After treatment,JOA scores of both groups increased(P<0.05),and the increase of observation group was higher than that of control group(P<0.05).After treatment,the levels of serum NO,IL-6,substance P and MMP-3 decreased in both groups(P<0.05),and the decrease degree in the observation group was higher than that in the control group(P<0.05).After treatment,lower limb sensory impairment score,lumbar motion and lumbar curvature in both groups increased(P<0.05),and the increase in observation group was higher than that in control group(P<0.05).Conclusion Bone-setting massage combined with acupuncture and moxibustion is effective in treating simple lumbar intervertebral disc herniation,which can obviously relieve pain and improve lower limb sensory disturbance.
