叙事护理在初产妇自然分娩应对中的影响

摘　　要：目的叙事护理应用于初产妇自然分娩以改善患者分娩体验的效果尚不明确。文章探讨叙事护理在初产妇自然分娩应对中的干预效果。方法采用非随机对照试验方法,将2019年2月至4月入住新沂市中医医院产科拟行自然分娩的初产妇60例设为对照组,接受常规产科自然分娩护理措施;将2019年5月至7月入科拟行自然分娩的初产妇60例设为试验组,在接受常规自然分娩护理基础上予以叙事护理干预。评估2组初产妇分娩疼痛程度、焦虑状况及产后分娩体验、首次母乳喂养成功率、总产程时长。结果经叙事护理干预,试验组潜伏期和活跃期疼痛评分[(5.80±0.75,7.88±0.76)分]均低于对照组[(6.33±0.51,8.51±0.50)分],差异均有统计学意义(P<0.05)。试验组活跃期焦虑评分[(45.88±4.72)分]低于对照组[(56.86±7.74)分],亦低于试验组入产房时[(55.27±8.63)分],差异均有统计学意义(P<0.05)。试验组产后分娩体验评分[(51.21±6.71)分]、总产程时长[(8.69±1.80)h]和首次母乳喂养成功率(87.50%)均优于对照组[(42.32±6.41)分、(10.24±2.03)h、63.16%],差异有统计学意义(P<0.05)。结论叙事护理在初产妇自然分娩应对中,能够有效缓解其分娩疼痛程度和焦虑状况、优化初产妇分娩体验,并对分娩结局中首次母乳喂养成功率的提高、总产程时长的缩短有一定积极意义。Objective To explore the influence of narrative nursing in the natural delivery of primiparas.Methods Non-randomized controlled trial method was adopted,60 primiparas admitted to our hospital from February to April 2019 for natural childbirth were selected as the control group.These primiparas accepted the routine obstetric natural childbirth nursing measures;Sixty primiparas admitted from May to July 2019 were selected as the experimental group,and narrative nursing intervention was given on the basis of routine natural childbirth nursing.The anxiety,pain,delivery experience,success rate of first breastfeeding and the total length of labor were evaluated.Results There were 7 cases of abscission.After intervention,the difference of pain scores in incubation period and active period between groups was statistically significant(P<0.05)(5.80±0.75,7.88±0.76)in experimental group,(6.33±0.51,8.51±0.50)in control group;the difference of antenatal anxiety score,postpartum delivery experience,success rate of first breastfeeding,total length of labor between groups was statistically significant(P<0.05),with anxiety score of(45.88±4.72)in experimental group,(56.86±7.74)in control group,and better than that at pre-admission(55.27±8.6),postpartum delivery experience score of(51.21±6.71)in experimental group,(42.32±6.41)in control group;total length of labor of(8.69±1.80)h in experimental group,(10.24±2.03)h in control group;first breastfeeding success rate of(87.50%)in experimental group,(63.16%)in control group.Conclusion Narrative nursing could effectively relieve the pain and anxiety of primipara,optimize the experience of primipara after delivery,and improve the success rate of first breastfeeding and shorten the total length of labor.
