0~3岁性早熟女童乳腺早发育、子宫卵巢发育情况的中西医病因及随访研究

摘　　要：目的探讨超声在0~3岁女童性早熟病因诊断中的应用价值。方法收集2017年1月—2019年11月82例0-3岁乳房早发育的女童,超声检查子宫卵巢的大小、容积、卵泡大小。另选0~3岁健康女童20例为对照组,超声检测乳腺、子宫卵巢大小及容积及尿LH、FSH水平,进行比较。结果乳房早发育幼儿乳头后方探及均可探及低回声区,根据乳腺及外阴发育情况进行Tanner分期,卵巢容积及卵泡大小均比对照组增大(P<0.05),病例组卵巢容积、卵泡大小随复查时间越长,卵泡大小、卵巢容积呈下降趋势。结论0~3岁女童尽管受微小青春期的影响,但是干预后增大的乳腺腺体范围和卵巢容积卵泡大小有下降趋势,说明干预因素在引起小儿性早熟中有一定的促进作用,及时干预,可以有效延缓性早熟的发生发展。Objective To evaluate the value of ultrasonography in etiological diagnosis of precocious puberty in 0~3 years old girls.Methods The size,volume,follicle size of uterus and ovary were examined by ultrasonography in 82 girls with early breast development from January 2017 to November 2019.Another 20 healthy girls aged 0~3 years old were selected as the control group.The size and volume of the breast,uterus and ovary were measured by ultrasound and compared with those of the case group.Results The hypoechoic area can be detected by the posterior probe of the nipple in early breast development children.Tanner staging was performed according to breast and vulvar development.The volume of ovary and the size of follicles in the case group were increased than those of control group(P<0.05).The ovarian volume and follicle size in the case group decreased with the time of reexamination.Conclusion Despite the influence of mini-puberty,the enlarged breast gland range and the volume of ovarian follicles decreased in 0~3 years old girls after intervention.It is suggested that intervention factors can promote the occurrence and development of precocious puberty.Timely intervention can delay the occurrence and development of precocious puberty.
