摘 要:目的通过对比脑瘫患儿下肢胫前肌表面肌电图i EMG值、足背伸角度、粗大运动功能评定(Gross Moto Function Measure,GMFM)量表,以及体内NF-κB、Bcl-2、Bax的含量,对电针及振动训练改善脑瘫患儿运动功能障碍的临床疗效对比,并从NF-κB信号调控肌细胞凋亡角度对其治疗机制进行探究。方法选取医院神经康复科收治痉挛型脑性瘫痪患者90例,随机分为三组,其中训练组予运动疗法等康复训练;电针训练组在康复训练基础上加用电针治疗;振动训练组在康复训练基础上振动训练治疗,所有治疗均1次/d,每周治疗5 d,12周为1个疗程。对比疗程前后患者胫前肌表面肌电图i EMG值、足背伸角度、GMFM量表以及血清NF-κB、Bcl-2、Bax含量。结果①治疗后电针训练组及振动训练组较治疗前胫前肌i EMG值显著提高,且治疗后较训练组胫前肌i EMG值显著提高,差异均有统计学意义(P<0.05);治疗后振动训练组较电针训练组i EMG值显著提高(P<0.05)。②治疗后电针训练组及振动训练组较治疗前胫前肌i EMG值显著提高(P<0.05);治疗后与训练组比较,电针训练组及振动训练组足背伸角度改善显著(P<0.05)。③治疗后训练组C区、电针训练组及振动训练组各区分值均较治疗前显著提高(P<0.05);治疗后与训练组比较,电针训练组C区、D区分值及振动训练组各区分值均有显著提高(P<0.05);治疗后与电针训练组比较,振动训练组D区分值显著提高(P<0.05)。④治疗后电针训练组及振动训练组血清NF-κB、Bcl-2含量较治疗前显著提高(P<0.05);治疗后电针训练组血清NF-κB、Bcl-2含量较训练组显著提高(P<0.05);且治疗后振动训练组血清NF-κB、Bcl-2含量较训练组及电针训练组均显著提高(P<0.05);治疗后3组血清Bax含量较治疗前均显著降低(P<0.05),且针灸训练组及振动训练组治疗后血清Bax含量较训练组显著降低(P<0.05)。结论电针治疗和振动训练可显�Objective To compare the clinical effects of electroacupuncture and vibration training on motor dysfunction in children with cerebral palsy by comparing the i EMG value of anterior tibial muscle of lower extremity,the extension angle of foot dorsum,the Gross Moto Function Measure(GMFM)scale and the contents of NF-κB,Bcl-2 and Bax in vivo,andregulate apoptosis of myocytes from NF-κB signal.To explore its treatment mechanism.Methods Ninety patients with spastic cerebral palsy were randomly divided into three groups.The training group was given exercise therapy and other rehabilitation training.The electroacupuncture training group was given electroacupuncture treatment on the basis of rehabilitation training.The vibration training group was given vibration training treatment on the basis of rehabilitation training.All were treated once a day,5 days a week,12 weeks as a course of treatment.The i EMG value,dorsal extension angle,GMFM scale and serum levels of NF-κB,Bcl-2 and Bax were compared before and after treatment.Results①After treatment,the i EMG of tibialis anterior muscle in the electroacupuncturetraining group and the vibration training group was significantly higher than that before treatment,and the i EMG of tibialis anterior muscle was significantly higher than that in the training group(P<0.05).After treatment,the i EMG of the vibration training group was significantly higher than that of the electroacupuncture training group(P<0.05).②After treatment,compared with that of the training group,the dorsal extension angle of foot in the electroacupuncture training group and the vibration training group was improved significantly(P<0.05).③After treatment,the difference values of training grouparea C,EA training group and vibration training group were significantly higher than those before treatment(P<0.05).After treatment,compared with that of the training group,the difference of C,D in the electroacupuncturetraining groupand vibration training groups was significantly higher(P<0.05).After treatment,c
