摘 要:目的评估江苏地区全血献血者铁营养状况。方法 2018年1—2月以及5月随机抽取全血献血者血液样品,采用生化指标评价铁营养状况。HemoCue测定血红蛋白(Hb),采用化学发光免疫分析法测定血铁4项,包括血清铁(SI),血清总铁结合力(TIBC),转铁蛋白(Tf)和血清铁蛋白(SF)。结果共调查995名全血献血者。Hb、SI、TIBC、TF和SF水平分别为140 g/L、19.2μg/L、69.4μg/L、2.1 g/L和134μg/L。铁缺乏检出率为7.8%(男性:SF<30μg/L,女性:SF<10μg/L)。男性和女性献血者铁缺乏率分别为3.3%(19/583)和14.3%(59/412),比较具有统计学意义。6.6%献血者具有较高的SF水平(SF>300μg/L)。献血者年龄与铁缺乏率之间无关联性。初次和再次献血者铁缺乏率比较具有统计学意义;再次献血者献血次数和献血频率与铁缺乏之间没有关联性。结论江苏地区全血献血者铁营养状况男性优于女性,建议高SF男性献血者缩短献血间隔期,提高临床供血量。育龄女性献血者需关注铁营养状况。Objective To evaluate the iron status of whole blood donors in Jiangsu province, China. Methods Blood samples of whole blood donors from January,February and May 2018 were randomly selected, and the iron status was evaluated via biochemical indicators. Hemoglobin(Hb) was measured by HemoCue 301.The iron state, including serum iron(SI), total iron binding capacity(TIBC), transferrin(Tf) and serum ferritin(SF), were measured using Chemiluminescence immunoassay(CLIA). Results A total of 995 whole blood donors were enrolled, and Hb, SI, TIBC, TF and SF showed mean values of 140 g/L, 19.2 μg/L, 69.4 μg/L, 2.1 g/L and 134 μg/L, respectively, for the blood donors. The overall incidence of iron deficiency(ID) was 7.8%, with the ID criteria as SF<30 μg/L in male and SF<10 μg/L in female. The sex of the blood donors was found to exert an influence on the incidence of ID in favor of the female(14.3%,59/412) than male(3.3%, 19/583). 6.6% of blood donors had a somewhat high SF level(SF>300 μg/L). No correlation was noticed between the age of donors and the incidence of ID. A significant trend of ID incidence was observed between first-time donors and repeat donors(P<0.05), rather than donation numbers or frequency of repeat donors(P>0.05).Conclusion The iron status of male whole blood donors is better than that of women in Jiangsu province. Male donors with high SF value are suggested to shorten blood donation intervals to elevate the collection units, while the iron status of women of childbearing age requires more attention.
