摘 要:目的为分离机制备浓缩血小板的原料血选择最佳离心参数、装罐方法,为各大采供血机构制备浓缩血小板(WBPC)的标准化研究提供参考。方法 1)将107袋健康全血随机分为不同离心参数的4组,分别为:A组第1步离心2 620 g×14 min,第2步离心236 g×10 min;B组第1步离心1 875 g×15 min,第2步离心277 g×6 min;C组第1步离心2 200 g×8 min,第2步离心1 220 g×5 min;D组第1步离心2 100 g×12 min,第2步离心400 g×8 min。通过对血小板计数、回收率、红细胞、白细胞的残留量、初次分离的血浆重量的比较选择最佳离心参数。2)将90袋白膜袋随机分为3组,每组30袋,将2次离心白膜袋与其所连配袋按实际装罐位置整齐摆放,测量高度p,计算空腔距离h=离心杯内径(8.5 cm)-白膜高度p,离心参数为236 g×10 min、(22±2)℃,刹车档加速9减速6。按空腔距离分组(A组:h=1—2 cm;B组:h=0—1 cm;C组:h=-1—0 cm)。进行WBC、RBC残留量、Plt、重量和回收率、回离率的比较。结果 1)B组的WBC残留量最多[(7.38±7.06)×106/L],与A、C、D组比较Ρ<0.05;C组Plt [(2.16±1.09)×1010/L]和血小板回收率[(34.03±16.70)%]最低,与A、B、D组比较Ρ<0.05;A组的血浆初次分离重量最高[(182.11±17.76)g],与C、D组比较Ρ<0.05;C组血浆回离率最高(81.3%),与A、B、D组比较Ρ<0.05。2)C组WBC残留量最多[(5.07±6.48)×106/L],与A、B组比较Ρ<0.05;B组Plt最高[(5.53±1.11)×1010/L],与A、C组比较Ρ<0.05;B组回收率最高[(69.96±16.70)%],与A、C组比较Ρ<0.05;B组回离率最低(3.3%),A组与B组回离率比较Ρ<0.05。结论重离心:2 620g×14 min,轻离心:236 g×10 min,(22±2)℃有更高的血小板回收率更低的白细胞残留和更高的出浆量,推荐给各采供血机构使用的离心参数。装罐时推荐控制空腔距离在0—1 cm,可以高效的回收血小板、降低WBC的残留且减少血小板2次回离的几率。Objective To select the optimal centrifuge parameters and filling method for buffy-coat pooled whole-blood-derived platelet concentrates(WBPCs), and to provide references for standardization study of WBPCs prepared by different blood banks. Methods 1) 107 bags of healthy whole blood were randomly divided into 4 groups, i.e. Group A, B, C and D, and subjected to two-step centrifugation(fisrt hard-and then soft-spin) with different centrifuge parameters: group A was centrifuged initially at 2 620 g×14 min then 236 g×10 min;group B at 1 875 g×15 min,then 277 g×6 min;group C at 2 200 g×8 min,then 1 220 g×5 min;group D at 2 100 g×12 min,then 400 g×8 min.The platelet count, yield rate, residual amount of RBC/WBC, and plasma weight after the initial centrifugation were compared to choose the optimal centrifuge parameters. 2) 90 bags of buffy coat bags were randomly divided into 3 groups(30 bags/group). The buffy coat products after second centrifugation were placed neatly according to the actual filling position with the matching bags, and the height(P) was measured. Cavity length(h) was equal to inner diameter of centrifuge cup(8.5 cm) minus buffy coat height(p). The centrifue parameters were set as 236 g×10 min at(22±2)℃, with the faster gear at 9 and slower gear at 6. Group A, B and C was classified by cavity length(h) :1—2 cm, 0—1 cm, and-1 cm—0 cm in group A, B and C, respectively. The count of WBC/RBC residues, Plt count, weight, yield rate and re-centrifuge rate were compared. Results 1) The leukocyte content(106/L) was highest in group B(7.38±7.06) as compared with group A, C, D(Ρ<0.05);group C exhibited the lowest recovery rate(%)(34.03±16.70) and highest re-centrifuge rate(81.3%) as compared with group A, B and D, yielding(2.16±1.09)×1010/L platelets, which was significantly lower than other 3 groups(Ρ<0.05);plasma weight after initial centrifugation was the highest in group A, with significant difference in comparison to group C and D(Ρ< 0.05). 2) The amount of WBC residues(×106/L) w
