摘 要:目的探讨献血者血液筛查标本经56℃水浴30min灭活后对HBsAg、抗-HCV、HIV Ab/Ag、抗-TP、HBV DNA、HCV RNA、HIV RNA和ALT检测的影响,比较分析灭活前后检测结果。方法随机抽取2020年2—6月份检验科报告结果均无反应性标本790份,HBsAg呈反应性标本116份,抗-HCV呈反应性标本46份,HIV Ag/Ab呈反应性标本62份,抗-TP呈反应性标本80份。HBV DNA反应性标本45份,HCV RNA反应性标本30份, HIV RNA反应性标本30份,随机抽取不同浓度ALT标本共120份。56℃水浴灭活30min前后酶免四项标本均采用STAR加样,FAME检测,收集各标本S/CO值;ALT项目标本用AU680生化仪检测,比较灭活前和灭活后平衡至不同时间段ALT活性。核酸标本56℃水浴灭活30min前后分别采用Cobas s201和Proleix Panther System核酸检测系统检测。结果热灭活前后,HBsAg、抗-HCV、抗-TP灰区标本水浴后S/CO值均>1,呈反应性,检测结果符合检测预期,HBV DNA、HCV RNA、HIV RNA检测结果均符合预期,定性结果和报告结果一致。但ALT检测结果有明显差异,具有统计学意义(P<0.01)。结论献血者血液筛查标本56℃水浴30min灭活后检测,会影响ALT检测的准确性,但不会影响酶免四项和核酸的定性结果。Objective To investigate the influence of 30-minute inactivation by 56℃ water bathing on the screening test results,including HBsAg, anti-HCV, HIV Ab/Ag, anti-TP, HBV DNA, HCV RNA, HIV RNA and ALT, of blood samples by comparing the detection results before and after inactivation. Methods 790 non-reactive samples, reactive samples in HBsAgs(n=116), anti-HCV(n=46), HIV Ag/Ab(n=62) and anti-TP(n=80), NAT reactive samples in HBV DNA(n=45), HCV RNA(n=30) and HIV RNA(n=30), and 120 ALT samples with different concentrations were randomly selected from February to June 2020.S/CO values of all samples before and after 30-minute inactivation by water bathing at 56℃ were collected by STAR sampling and FAME detection. ALT samples were detected by AU680 biochemical analyzer to compare ALT activity at different time periods before and after inactivation. Cobas s201 and Proleix Panther System were used to detect NAT samples before and after inactivation. Results S/CO values of all HBsAg, anti-HCV and anti-TP grey area samples remained> 1 after water bathing, presenting expected test results. And the NAT results of HBV DNA, HCV RNA and HIV RNA samples all meet the expectations and the qualitative results were consistent with the reported results. However, ALT results were significantly different, with statistical significance(P<0.01). Conclusion Water bathing at 56℃ for 30 min could affect the accuracy of ALT detection, but not the qualitative results of ELISA and NAT.
