从“肺朝百脉、主治节”探讨慢性心力衰竭心室重塑

摘　　要：心力衰竭是一个复杂的进行性病变,发生机制尚未完全阐明。学者普遍认为神经-体液调节失衡是其关键途径,炎症机制、细胞凋亡等因素共同参与引起心肌舒缩功能障碍,从而导致心衰的发生,其中心室重塑是慢性心力衰竭发生发展的重要机制之一。在中医理论中气虚痰瘀内阻是慢性心衰时心室重塑的主要病机,《黄帝内经》提出“肺朝百脉、主治节”;肺主呼吸之气,主宗气生血;肺主宣发肃降行血;肺朝百脉,主治节调血;通调水道利血行,因此提出“肺朝百脉、主治节”对丰富和完善中医藏象理论及指导慢性心力衰竭心室重塑的防治具有重要意义。Heart failure is a complex and progressive disease,the mechanism of which has not been fully elucidated.It is generally believed that the imbalance of neurohumoral regulation is the key pathway,and the inflammatory mechanism,apoptosis and other factors are involved in the cardiac systolic and diastolic dysfunction,which leads to the occurrence of heart failure.Ventricular remodeling is one of the important mechanisms of the occurrence and development of chronic heart failure.In the theory of traditional Chinese(TCM)medicine,we believe that Qi deficiency,phlegm and blood stasis are the basic pathological mechanisms of ventricular remodeling in chronic heart failure.The formation of this mechanism is closely related to the dysfunction of“convergence of vessels in the lung“and“lung governing coordinative activities of viscera”.In TCM theory,the lung is the main source of breath and blood;the lung is the direction of hundreds of blood vessels;the lung is the main source of regulating blood;the lung is the channel of regulating water and promoting blood circulation.Therefore,it is of great significance for enriching and perfecting the theory of TCM viscera and guiding the prevention and treatment of ventricular remodeling in chronic heart failure.
