怡心宁神汤治疗冠状动脉粥样硬化性心脏病经皮冠状动脉介入治疗术后合并抑郁症的临床观察

摘　　要：目的:观察怡心宁神汤治疗冠状动脉粥样硬化性心脏病经皮冠状动脉介入治疗(Percutaneous Coronary Intervention,PCI)术后合并抑郁症的临床疗效。方法:选择符合纳入标准的冠状动脉粥样硬化性心脏病PCI术后合并抑郁症患者60例,按随机数字表法分为对照组和治疗组各30例,对照组口服盐酸氟西汀胶囊,治疗组采用怡心宁神汤进行治疗。比较观察两组治疗4周后患者汉密尔顿抑郁量表(Hamilton Depression Scale,HAMD)评分、临床症状、中医证候比较及药物不良反应,并评定临床疗效。结果:对照组显效13例,有效14例,无效3例,总有效率为90.0%(27/30),治疗组显效15例,有效13例,无效2例,总有效率为93.3%(28/30),两组疗效比较,差异无统计学意义(P>0.05);两组HAMD量表评分比较,治疗前差异无统计学意义(P>0.05),治疗后差异有统计学意义(P<0.05),治疗组HAMD评分低于对照组,治疗组具有明显改善;两组中医证候比较,治疗组中医证候改善情况优于对照组,差异有统计学意义(P<0.05)。结论:怡心宁神汤对改善冠状动脉粥样硬化性心脏病PCI术后合并抑郁症患者疗效确定,不良反应少,安全性较好。Objective:To observe the clinical effect of Yixin Ningshen decoction(怡心宁神汤)in the treatment of coronary heart disease complicated with depression after PCI.Methods:60 patients with coronary heart disease and depression after PCI were divided into the control group and the treatment group according to the random number table.Fluoxetine hydrochloride capsule was orally taken in the control group and Yixin Ningshen decoction was used in the treatment group.The HAMD scores,clinical symptoms,TCM symptoms and adverse drug reactions of the two groups were compared and evaluated.Results:In the control group,13 cases were significantly effective,14 cases were effective,3 cases were ineffective,and the effective rate was 90.0%(27/30).In the treatment group,15 cases were significantly effective,13 cases were effective,2 cases were ineffective,and the effective rate was 93.3%(28/30).There was no significant difference between the two groups(P>0.05).There was no significant difference between the two groups before treatment(P>0.05).The HAMD score of the treatment group was lower than that of the control group after treatment,and the treatment group was significantly improved;the TCM syndrome improvement of the treatment group was better than that of the control group,the difference was statistically significant(P<0.05).Conclusion:Yixin Ningshen decoction has definite curative effect on the patients with depression after PCI of coronary heart disease,it has less adverse reactions and better safety.
