益心汤联合美托洛尔治疗慢性收缩性心力衰竭的临床研究

摘　　要：目的:探讨益心汤联合酒石酸美托洛尔治疗慢性收缩性心力衰竭的临床疗效。方法:选取北京市通州区宋庄卫生院2017年12月-2018年12月诊治的慢性收缩性心力衰竭患者158例,采用随机数字表法分为两组,对照组患者79例采用酒石酸美托洛尔治疗,观察组患者79例采用益心汤联合酒石酸美托洛尔治疗,于治疗前后行心功能评定、生化指标检测、中医证候评分和日常生活能力评分,比较两组患者的临床疗效、不良反应。结果:两组患者治疗后左室射血分数、6 min步行距离、日常生活能力评分较治疗前显著增加(P<0.05)。两组患者治疗后左室舒张末期内径、左室收缩末期内径、N末端脑钠肽原、全血高切黏度、全血低切黏度、血浆黏度、中医证候评分较治疗前显著降低(P<0.05)。观察组患者治疗后左室射血分数、6 min步行距离、日常生活能力评分高于对照组(P<0.05)。观察组患者治疗后左室舒张末期内径、左室收缩末期内径、N末端脑钠肽原、全血高切黏度、全血低切黏度、血浆黏度、中医证候评分低于对照组(P<0.05)。观察组患者总有效率高于对照组(P<0.05)。结论:益心汤联合酒石酸美托洛尔治疗慢性收缩性心力衰竭的疗效显著,值得临床推广使用。Objective:To investigate the clinical effect of Yixin decoction(益心汤)combined with metoprolol in treating chronic systolic heart failure.Methods:158 patients with chronic systolic heart failure were selected in Songzhuang health center,Tongzhou district,Beijing,from December 2017 to December 2018,who were divided into two groups by random number table method.79 patients treated with metoprolol were the control group.79 patients treated with Yixin decoction combined with metoprolol were the observation group.Cardiac function evaluation,biochemical index test,TCM syndrome score and daily living ability score were evaluated before and after treatment.The clinical efficacy and adverse reactions were compared between two groups.Results:Left ventricular ejection fraction,6-minute walking distance,daily living ability score increased significantly in two groups after treatment(P<0.05).Left ventricular end diastolic diameter,left ventricular end systolic diameter,NT-Pro BNP,whole blood high shear viscosity,whole blood low shear viscosity,plasma viscosity,TCM syndrome score decreased significantly in two groups after treatment(P<0.05).Left ventricular ejection fraction,6-minute walking distance,daily living ability score in the observation group were higher than those of the control group after treatment(P<0.05).Left ventricular end diastolic diameter,left ventricular end systolic diameter,NT-Pro BNP,whole blood high shear viscosity,whole blood low shear viscosity,plasma viscosity,TCM syndrome score in the observation group were lower than those of the control group after treatment(P<0.05).The total effective rate in the observation group was higher than that of the control group(P<0.05).Conclusion:Yixin decoction combined with metoprolol has significant effect in treating chronic systolic heart failure,which is worthy of clinical application.
