摘 要:目的:分析膈肌肌电图在慢性阻塞性肺疾病(COPD)患者膈肌疲劳检测中的应用效果。方法:回顾性分析2017-2019年120例COPD膈肌疲劳患者的临床资料作为观察组,选择同期120例健康体检者作为对照组。均行膈肌肌电图检测,比较两组频谱指标[中心频率(Fc)、高频(H)/低频(L)值]水平、膈神经传导时间(PNCT)及运动单位动作电位(MUAP)幅度水平。结果:观察组治疗前后的Fc、H/L均明显低于对照组,且观察组治疗后的Fc和H/L均明显高于治疗前,差异有统计学意义(P<0.05);治疗前后观察组左侧及右侧PNCT均明显长于对照组,左侧及右侧MUAP均明显低于对照组,且治疗后观察组左侧及右侧PNCT均明显短于治疗前,左侧及右侧MUAP均明显高于治疗前,差异均有统计学意义(P<0.05)。结论:膈肌肌电图检测能反映COPD患者膈肌疲劳程度,可为临床判断、评估病情提供依据。Objective:To explore application effects of diaphragmatic electromyogram in detection of diaphragmatic fatigue in patients with chronic obstructive pulmonary disease(COPD).Methods:The clinical data of 120 COPD patients with diaphragmatic fatigue from 2017 to 2019 were retrospectively analyzed,and these patients were used as observation group.120 healthy subjects in the same period were used as control group.Diaphragmatic electromyography was performed for the two groups.The spectrum indicators[central frequency(Fc),high frequency(H)/low frequency(L)value]levels,phrenic nerve conduction time(PNCT)and motor unit action potential(MUAP)amplitude levels were compared between the two groups.Results:The Fc and H/L of the observation group before and after the treatment were significantly lower than those of the control group;the Fc and H/L of the observation group after the treatment were significantly higher than those before the treatment;and the differences were statistically significant(P<0.05).The left and right PNCTs of the observation group before and after the treatment were significantly longer than those of the control group,and the left and right MUAPs were significantly lower than those of the control group;the left and right PNCTs of the observation group after treatment were significantly shorter than those before the treatment,and the left and right MUAPs were significantly lower than were significantly higher than those before the treatment;and the differences were statistically significant(P<0.05).Conclusions:Diaphragmatic electromyogram detection can reflect the degree of diaphragmatic fatigue in the COPD patients,and can provide a basis for clinical judgment and evaluation of the condition.
