外用痔疮膏及硫酸镁联合应用经皮穴位电刺激治疗产后外痔的临床疗效观察

摘　　要：目的外用痔疮膏及硫酸镁联合应用经皮穴位电刺激(TAES)治疗产后外痔,观察其临床疗效,评估对痔的改善作用,并分析其作用机制。方法选择2017年9月—2018年11月100例产后外痔产妇,随机分为对照组和研究组,每组50例。对照组均给予外用痔疮膏、硫酸镁湿敷治疗,研究组在对照组治疗的基础上应用TAES进行治疗。评估两组患者治疗后临床疗效,分别于治疗前和治疗5 d后评估外痔水肿体积、痔疮评分,采用即视觉模拟评分法(VAS)评估痔疮局部创面疼痛程度,评估产后子宫恢复、母乳喂养情况及满意度。结果研究组总有效率、子宫复旧良好率、母乳喂养率、满意度评分均高于对照组(P<0.05);研究组治疗后水肿体积、VAS评分和痔疮评分低于对照组(P<0.05),差异有统计学意义。结论家庭康复联合应用TAES治疗产后外痔,临床疗效显著,能够显著减轻痔的疼痛和水肿,并能促进子宫复位,提高母乳喂养率。Objective On the basis of topical hemorrhoid cream and magnesium,combined with percutaneous acupoint electrical stimulation(TAES) in the treatment of postpartum external hemorrhoids in family rehabilitation,observe its clinical efficacy,evaluate the improvement effect on sputum,and analyze its mechanism of action.Methods 100 cases of postpartum external hemorrhoids from September in 2017 to November in 2018 were selected and randomly divided into control group and study group,50 cases in each group.The control group was treated with topical hemorrhoid cream and magnesium sulfate wet compress,the study group also applied TAES treatment.Assessed the clinical efficacy of the patient.Acne edema volume and acne score were assessed before treatment and 5 days after treatment.The visual analog scale(VAS) was used to evaluate the degree of pain in localized acne,and the postpartum uterus recovery,breastfeeding and satisfaction were evaluated.Results The total effective rate uterine recovery rate,breastfeeding rate and satisfaction score of the study group were 96.00%(48/50),76.00%(38/50),78.00%(39/50) and(96.73 ± 3.28) points respectively,higher than the control group 84.00%(42/50),66.00%(33/50),68.00%(34/50) and(90.42 ± 3.15) points,the difference was statistically significant(P<0.05).The edema volume,VAS score and acne score of the study group were(92.37 ± 16.05) mm,(2.46 ± 0.25) and(6.22 ± 0.93) respectively,lower than the control group(124.80 ± 16.93)mm,(3.54 ± 0.28) was divided into(7.94 ± 0.86) points,and the difference was statistically significant(P<0.05).Conclusions TAES treatment of postpartum external hemorrhoids has the significant clinical effect,which can significantly reduce the pain and edema of sputum and promote uterine reduction and increase the rate of breastfeeding.
