摘 要:目的研究低强度脉冲电磁场(pulsed electromagnetic fields,PEMFs)对SD大鼠跟腱异位骨化(HO)的影响。方法60只SD大鼠按随机数字表法分为空白手术组、HO模型组、PEMFs干预组、吲哚美辛干预组,各15只。除空白手术组外均采用血管钳反复钳夹跟腱建立跟腱HO模型大鼠。PEMFs干预组(PEMFs照射患肢1天2次,每次20min)、吲哚美辛干预组(10mg·kg-1·d-1剂量吲哚美辛混悬液4mL灌胃)相应干预12周。X线片观察大鼠跟腱部位新骨形成,跟腱标本行苏木精-伊红(HE)染色和Masson染色进行组织学观察,免疫组织化学(IHC)检测和蛋白质印迹(Western bolt)分析检测环氧合酶2(COX-2)和骨形态发生蛋白2(BMP-2)表达。结果血管钳反复钳夹跟腱成功构建跟腱HO模型大鼠。PEMFs干预组和吲哚美辛干预组大鼠术后12周X线片跟腱骨化面积低于模型组;HE和Masson染色结果显示,PEMFs干预组和吲哚美辛干预组大鼠跟腱细胞肌腱排列整齐,无软骨细胞和骨组织;IHC和Western blot的结果显示,HO模型组大鼠跟腱软骨细胞聚集区的COX-2和BMP-2表达较空白手术组增多[COX-2:(4.167±0.983)分比(1.0±0.632);BMP-2:(5.0±1.095)分比(1.833±0.753)分,P<0.01],PEMFs干预组和吲哚美辛干预组COX-2、BMP-2表达较HO模型组明显降低[COX-2:(2.833±0.983)分、(2.167±1.169)分比(4.167±0.983)分;BMP-2:(3.0±1.095)分、(2.5±1.225)分比(5.0±1.095)分,P<0.05,P<0.01]。结论PEMFs干预能有效预防大鼠HO的发生。Objective To investigate the effects of low-intensity pulsed electromagnetic fields(PEMFs)on the heterotopic ossification(HO)of the Achilles tendon in SD rats.Methods Sixty SD rats were randomly divided into blank operation group,HO model group,PEMFs intervention group and indomethacin intervention group,with 15 animals in each group.Except the blank operation group,the Achilles tendon HO model was established by repeatedly clamping the Achilles tendon with vascular clamp.PEMFs intervention group(i.e.,PEMFs irradiation of affected limb twice a day,20min each time)and indomethacin intervention group(i.e.,10mg·kg-1·d-1 dose of indomethacin suspension 4mL by gavage)were intervened for 12 weeks.The new bone formation in the Achilles tendon of rats was observed by X-ray,and the tendon samples were stained with hematoxylin-Eosin(HE)and Masson for histological observation,and the expressions of cyclooxygenase-2(COX-2)and bone morphogenetic protein 2(BMP-2)were detected by immunohistochemistry(IHC)and Western bolt.Results The Achilles tendon ho model was successfully established by repeatedly clamping the Achilles tendon with vascular clamp.The ossification area of Achilles tendon on X-ray film in PEMFs intervention group and indomethacin intervention group was lower than that in model group at 12 weeks after operation.HE and Masson staining showed that the tendon of Achilles tendon cells in PEMFs intervention group and indomethacin intervention group was arranged neatly without chondrocytes and bone tissue.IHC and Western blot results showed that COX-2 and BMP-2 in the aggregation area of Achilles tendon chondrocytes in HO model group were significantly lower than those in model group,COX-2:[(4.167±0.983)vs(1.0±0.632)],BMP-2:[(5.0±1.095)vs(1.833±0.753),P<0.01].The expression of COX-2 and BMP-2 in PEMFs and indomethacin intervention groups was significantly lower than that in HO model group,i.e.,COX-2:[(2.833±0.983)and(2.167±1.169)vs(4.167±0.983)],BMP-2:[(3.0±.095)and(2.5±1.225)vs(5.0±1.095),P<0.05,P<0.01,res
