艾拉莫德降低类风湿性关节炎患者血清抗环瓜氨酸肽抗体和类风湿因子滴度

摘　　要：目的探讨艾拉莫德联合甲氨喋呤治疗对类风湿性关节炎(RA)患者血清抗环瓜氨酸肽抗体(ACPA)和类风湿因子(RF)滴度的影响。方法66例活动性RA患者中,30例应用艾拉莫德联合甲氨喋呤治疗(A组),36例应用来氟米特联合甲氨喋呤治疗(B组)。记录两组治疗前后血常规、肝肾功能、ESR和CRP水平以及ACPA和RF滴度变化,并观察治疗过程中不良反应的发生情况。结果A组治疗后血清ACPA和RF滴度以及CRP和ESR水平均较治疗前降低(P<0.05);B组治疗后CRP和ESR水平较治疗前降低(P<0.05),但血清ACPA和RF滴度与治疗前比较差异无统计学意义(P>0.05)。两组患者治疗前后外周血WBC总数、Plt计数、ALT、TBil及SCr水平比较差异均无统计学意义(P>0.05);A组治疗后Hb水平较治疗前升高(P<0.05)。两组均无严重不良反应发生。结论艾拉莫德联合甲氨喋呤治疗可有效降低RA患者血清ACPA和RF滴度,且安全性较高。Objective To investigate the effect of iguratimod combined with methotrexate on the titers of serum anti-cyclic citrullinated peptide antibody(ACPA)and rheumatoid factor(RF)in the patients with rheumatoid arthritis(RA).Methods Of 66 patients with active RA,30 cases(group A)were treated with iguratimod combined with methotrexate,and 36 cases(group B)were treated with leflunomide combined with methotrexate.The complete blood count,hepatic and renal function,levels of ESR and CRP and titers of ACPA and RF were recorded before and after treatment.The adverse responses were observed during the treatement.Results Compared with before,the titers of ACPA and RF and levels of ESR and CRP were all significantly decreased after treatment in group A(P<0.05).The levels of ESR and CRP after treatment were lower than those before treatment in group B(P<0.05),but there was no statistical difference in the titers of ACPA and RF before and after treatment(P>0.05).The total number of WBC,Plt count,ALT,TBil and SCr after treatment were similar to those before treatment in the both groups(P>0.05).While the level of Hb after treatment was higher than that before treatment in group A(P<0.05).No serious adverse responses occurred in both groups.Conclusion Iguratimod combined with methotrexate could effectively reduce the titers of serum ACPA and RF with higher security in the patients with RA.
