四联序贯疗法用于环状混合痔术后并发症的临床观察

摘　　要：为观察四联序贯疗法用于环状混合痔术后并发症的临床疗效及对患者生活质量的影响,将100例环状混合痔患者随机分成观察组和对照组,各50例。观察组术后当天即采用四联序贯疗法(地佐辛泵注、利多卡因湿敷、按摩配合穴位注射、利多卡因混合开塞露肛门注入)治疗,对照组采用常规术后对症治疗。观察比较2组术后疼痛、排便困难、腹胀、水肿等并发症,及患者焦虑、睡眠质量、生活质量等情况。结果显示,观察组术后第1,3,5,7天疼痛积分均明显优于对照组(P<0.05);首次排尿时间和平均显效时间均短于对照组(P<0.05);并发症发生率明显低于对照组(P<0.05);术后第1天及第7天患者焦虑、睡眠质量和生活质量评分均优于对照组(P<0.05)。结果表明,环状混合痔患者术后运用四联序贯疗法治疗,能显著改善术后疼痛、尿潴留等情况,减少排便困难、腹胀等术后并发症的发生率,可减轻手术对患者情绪、睡眠和生活质量的影响。This study was to observe the clinical effect of quadrigeminal sequental therapy(QST)for postoperative complications of circumferential mixed hemorrhoids(CMH),and its influence on patients’quality of life,randomly divided 100 CMH patients into observation group(50 cases,on the operative day adotped the QST,i.e pump-infusing dezocine,moisture dressing lidocaine,massage plus point-injection,as well rectal administration of lidocaine plus open plug dew)and control group(50 cases,postoperativelly routine target treatment);then,observed and compared both groups’complication such was postoperative pain,dyschesia,edema and burbulence;and,patients’anxiety,sleep quality,and quality of life,etc.As results,in the VAS ratings on pain on the 1 st,3 rd,5 th and 7 th after surgery,and on patients’anxiety,sleep quality and quality of life on the 1 st and 7 th day after surgery;in the time for first urination and average onset time;and,in complication incidence observation group was respectively and significantly superior to,shorter and lower than control group(all,P<0.05).Results show that after surgery for CMH adopting QST can significantly improve postoperative complication such as pain,uroschesis,etc,reduce the incidence of dyschesia and burbulence,etc,also decrease the influence on patients’emotion,sleep and quality of life.
