摘 要:目的:探求危重型新型冠状病毒肺炎(COVID-19)患者血清肌钙蛋白I(cTnI)、肌酸激酶同工酶(CK-MB)、N末端B型利钠肽前体(NT-proBNP)与心功能指标的相关关系.方法:选取深圳市第三人民医院危重型COVID-19患者5例,采用发光法检测5例患者血清cTnI、CK-MB、NT-proBNP质量浓度,同时对应行心脏彩超检测左心室内径(LV)、左心室射血分数(LVEF)、肺动脉收缩压(PASP).采用SPSS 26.0的Pearson检验分析cTnI、CK-MB、NT-proBNP质量浓度与LV、LVEF、PASP的相关性.结果:危重型COVID-19患者cTnI、CK-MB、NT-proBNP均与LVEF呈负相关(r=-0.574,-0.758,-0.590,P<0.05);CK-MB、NT-proBNP均与PASP呈正相关(r=0.513,0.469,P<0.05),且与LV呈正相关(r=0.651,0.380,P<0.05).结论:危重型COVID-19患者可发生心脏损伤,cTnI、CK-MB、NT-proBNP质量浓度与心功能指标密切相关,其中CK-MB相关性最高;cTnI、CK-MB、NT-proBNP的异常升高可提示危重型COVID-19患者心脏功能或结构的恶化.Objective:To investigate the relationship between serum troponin I(cTnI),creatine kinase isoenzyme(CK-MB),N-terminal B-type natriuretic peptide precursor(NT-proBNP)and cardiac function in critical patients with Corona Virus Disease 2019(COVID-19).Methods:Five critical patients with COVID-19 in Shenzhen Third People’s Hospital were selected.The levels of serum cTnI,CK-MB and NT-proBNP were measured by luminescence method,and the left ventricular diameter(LV),left ventricular ejection fraction(LVEF)and pulmonary artery systolic pressure(PASP)were measured by color Doppler echocardiography.The Pearson test was used to analyze the correlation between the levels of cTnI,CK-MB,NT-proBNP and LV,LVEF,PASP by using software SPSS 26.0.Results:In critical patients with COVID-19,the levels of cTnI,CK-MB and NT-proBNP were negatively correlated with LVEF(r=-0.574,-0.758 and-0.590,P<0.05);those of CK-MB and NT-proBNP were positively correlated with PASP(r=0.513,0.469,P<0.05)as well as LV(r=0.651,0.380,P<0.05).Conclusion:Heart injury may occur in critical patients with COVID-19.The levels of cTnI,CK-MB and NT-proBNP are closely related to cardiac function,among which CK-MB has the greatest correlation.The abnormal increase of cTnI,CK-MB and NT-proBNP may indicate the deterioration of cardiac function or structure in critical patients with COVID-19.
