黏膜下挖除术与黏膜下剥离术切除胃固有肌层起源上皮下肿瘤的对比研究

摘　　要：目的探讨内镜黏膜下挖除术与黏膜下剥离术治疗胃固有肌层起源上皮下肿瘤的疗效差异。方法纳入2017年至2019年因胃固有肌层SETs在我院行内镜下手术的82例患者,其中42例接受粘膜下挖除术,40例接受粘膜下剥离术。比较ESE与ESD的整块切除率、手术时间、出血率等指标。结果ESE组的手术时间明显短于ESD组(P<0.01)。ESD组术后并发症发生率高于ESE组。结论ESE切除固有肌层起源的胃SETs的手术时间短、并发症发生率低,其安全有效性优于ESD。此外,内镜下切除起源于胃固有肌层深层、不规则形状肿物的手术风险相对高。Objective Investigating the efficacy of endoscopic submucosal excavation and dissection to treat gastric myogenic SETs.Methods We included 82 patients underwent endoscopic resection of gastric LAMINA PROPRIA SETs from 2017 to 2019.42 cases underwent ESE and 40 cases underwent ESD.Resection rate,time and bleeding rate were compared.Results The operation time of ESE was shorter than ESD(P<0.01).The postoperative complication incidence in ESD was higher than ESE.Conclusion ESE is a safe and effective method to treat gastric SETs.In addition,the risk of endoscopic resection of a deep,irregular shape mass originating from the gastric Lamina propria is relatively high.
