不同剂量阿托品应用于老年有机磷中毒患者对其血清酶学及胆碱酯酶变化的影响研究

摘　　要：目的探讨不同剂量阿托品应用于老年有机磷中毒患者以及对其血清酶学及胆碱酯酶变化的影响。方法随机选取2014年2月-2019年2月期间接诊的99例有机磷中毒老年患者,根据阿托品用量不同分为A组:33例,大剂量;B组:33例,中等剂量,C组:33例,小剂量,对所有患者实施综合治疗与处理,分析不同患者血清酶学及胆碱酯酶变化、治疗效果。结果三组患者治疗后较治疗前各项血清丙氨酸氨基转移酶(ALT)、乳酸脱氢酶(LDH)、肌酸激酶(CK)、门冬氨酸氨基转移酶(AST)明显下降,同时胆碱酯酶水平也显著下降,存在统计学意义(P<0.05),B组较A组、C组对应血清CK、AST、胆碱酯酶水平更低,存在统计学意义(P<0.05);B组较A组、C组患者存活率更高,差异有统计学意义(P<0.05)。结论老年有机磷中毒患者使用中等剂量阿托品,能够有效降低血清酶学及胆碱酯酶,提高患者存活率。Objective To investigate the effect of different doses of atropine on serum enzymology and cholinesterase in elderly patients with organophosphorus poisoning.Methods a total of 99 elderly patients with organophosphorus poisoning admitted from February 2014 to February 2019 were randomly selected and divided into group A:33 cases,high dose;group B:33 cases,medium dose;group C:33 cases,low dose.The comprehensive treatment and treatment were implemented for all patients,and the changes of serum enzyme and cholinesterase and treatment effect of different patients were analyzed.Results After treatment,serum alanine aminotransferase(ALT),lactate dehydrogenase(LDH),creatine kinase(CK),aspartate aminotransferase(AST)in three groups were significantly lower than those before treatment,and the levels of cholinesterase were also significantly decreased(P<0.05).The levels of CK,AST and cholinesterase in group B were lower than those in group A and group C The survival rate of group B was higher than that of group A and group C(P<0.05).Conclusion the use of moderate dose atropine in elderly patients with organophosphorus poisoning can effectively reduce serum enzyme and cholinesterase,and improve the survival rate of patients.
