PDCA循环法在某院门急诊处方点评中的持续改进与应用

摘　　要：目的探讨PDCA循环法在处方点评方面的应用及对临床合理用药的影响。方法2018年9月—2019年8月基于PDCA循环法对处方点评处在的问题进行管理,找出不合理处方的具体问题,分析原因,制定改进措施并保持持续改进。结果进行第一次PDCA循环后(2019年1月—4月vs 2018年9月—12月)处方合格率差异无统计学意义(χ2=3.796,P>0.05),其中不良药物相互作用和用法用量不适宜两种不合理处方类型差异具有统计学意义(χ2分别为17.911和24.118,P<0.05);继续进行第二次PDCA循环后(2019年5月—8月vs 2019年1月—4月)处方合格率差异有统计学意义(P<0.05),用法用量不适宜和配伍禁忌两种不合理处方类型差异具有统计学意义(χ2分别为10.696和4.143,P<0.05)。结论PDCA循环法在处方点评的持续改进方面取得了显著成效,符合医疗质量持续改进的要求,能更好的促进临床合理用药。Objective To study the application of PDCA cycle management method in prescription review and its influence on the rational usage of drugs.Methods From September 2018 to August 2019,based on the PDCA cycle method,the problems of prescription reviews are managed,the specific problems of unreasonable prescriptions are found out,the reasons are analyzed,improvement measures are formulated and continuous improvement is maintained.Results After the first PDCA cycle(from January 2019 to April 2019 compared with September 2018 to December 2018),there was no significant difference in the qualified rate of prescriptions(χ2=3.796,P>0.05),and there was significant difference in the drug interaction and the inappropriate dosage(χ2=17.911 and 24.118,P<0.05),continuing the second PDCA cycle(from May 2019 to August 2019 compared with January 2019 to April 2019),there was significant difference in the passing rate of prescriptions(χ2=10.040,P<0.05),there were significant differences between the two types of irrational prescriptions(χ2=10.696 and 4.143,P<0.05).Conclusion The PDCA cycle method has achieved significant results in the continuous improvement of prescription reviews,which meets the requirements of continuous improvement in medical quality,and can better promote rational drug use in the clinic.
