腹腔镜手术治疗小儿嵌顿性腹股沟斜疝疗效观察

摘　　要：目的:探讨腹腔镜手术与传统开放手术治疗小儿嵌顿性腹股沟斜疝的疗效及安全性。方法:回顾性分析82例嵌顿性腹股沟斜疝患儿的临床资料,其中腹腔镜手术组38例,传统开放手术组44例,比较2组患者手术时间、术中出血量、术后排气时间、术后住院时间、术后并发症、术后复发率等指标。结果:82例手术均顺利完成,腹腔镜组术中出血量少于传统手术组[(2.32±1.70)vs(9.51±7.52) mL],手术时间短于传统手术组[(33.80±16.71)vs(47.52±22.33) min],术后排气时间较传统手术组明显缩短[(12.32±3.84)vs(21.60±9.64) min],术后住院时间较传统手术组明显缩短[(3.83±1.62)vs(5.61±2.10) d],差异均具有统计学意义(P<0.05);术后均进行为期1年随访,手术并发症比较差异无统计学意义(P>0.05)。结论:腹腔镜手术治疗小儿嵌顿性腹股沟斜疝手术时间短,创伤小,恢复快,安全可行。Objective: To explore the efficacy and safety of laparoscopic surgery and traditional open surgery in the treatment of incarcerated inguinal hernia in children. Methods: The clinical data of 82 children with incarcerated inguinal hernia were retrospectively analyzed, including 38 cases in the laparoscopic surgery group, 44 patients in the traditional inguinal oblique incision open surgery group. Operative time, amount of bleeding, postoperative ventilation time and postoperative hospitalization time, postoperative complications and recurrence rate were compared between the two groups with t-test. Results: 82 surgeries were successfully completed. Compared with the traditional operation group, the laparoscopic operation group had better operation effect, the amount of bleeding was significantly reduced loss, the operation time was significantly shortened, the postoperative ventilation time was significantly shortened(P<0.05), the postoperative hospital stay was significantly shortened. All patients were followed up for one year after operation. There was no significant difference in the incidence and recurrence rate of complications(incision infection, scrotal pneumatosis and scrotal hematoma) between these two operations. Conclusions: Laparoscopic surgery for children with incarcerated indirect inguinal hernia has shorter postoperative rehabilitation time, less operative trauma, quick recovery, and is safe and feasible.
