上海市某区首例新型冠状病毒肺炎病例的处置分析

摘　　要：目的对某区首例新型冠状病毒肺炎的流行病学调查处置进行分析,为今后此类新发呼吸道传染病防控提供参考。方法收集整理病例流行病学调查、检测等资料,通过描述性流行病学方法进行分析。结果病例主动至发热门诊就诊,因接触过有呼吸道感染症状的温州朋友,诊断为新型冠状病毒肺炎疑似病例;经市疾控中心检出病例标本SARS-COV-2病毒核酸阳性后,转至市级定点医院治疗,于2月12日治愈出院。病例的温州朋友两次核酸检测阴性,最终未确诊,但其有确诊患者接触史。病例6名在沪家庭密接者经14天医学观察均未发病。结论面对新发呼吸道传染病,为及时发现病例,临床可结合疫情形势适当放宽诊断标准;病例自我隔离意识的提高可避免家庭聚集性疫情的发生;省市间病例接触史等信息的及时沟通有助于做出正确临床判断。Objective To analyze the epidemiological investigation and treatment of the first COVID-19 case in Qingpu District,provide a reference for the prevention and control of such kind of new infectious diseases in the future,especially respiratory infectious diseases.Methods Date such as case epidemiological investigation and treatment,laboratory testing were collected and analyzed through descriptive epidemiological methods.Results The patient went to the fever clinic for consultation initiatively.Due to the contraction with a friend from Wenzhou who had symptoms of respiratory tract infection,and combined with clinical manifestations,the district hospital diagnosed a suspected case of COVID-19.After the SARS-COV-2 virus was detected by the municipal center for disease control and prevention nucleic acid positive,this patient was transferred to a municipal designated hospital for treatment and discharged on Feb.12.Although the friend from Wenzhou had a negative nucleic acid test twice and the diagnosis is not confirmed,he had a history of contact with the diagnosed patient.Seven cases of close contacts in Shanghai did not get sick after 14 days of home isolation and medical observation.Conclusion In the face of new respiratory infectious diseases,medical staff can combine with the prevailing situation and appropriately relax the diagnostic criteria to find cases timely.Raising residents’awareness of home disease prevention through publicity,especially the self-isolation awareness of cases can avoid the occurrence of family epidemic outbreaks and reduce the formation of“family-community”transmission model.Prompt communication of information about case contact history between provinces and cities can help the clinics make the right judgment at the first time.
