摘  要:目的探讨定量非对称回波的最小二乘估算法迭代水脂分离序列(IDEAL-IQ)定量评价老年人全脊柱椎体骨髓脂肪含量的价值。方法选择2019年6月1日至7月28日杭州市萧山区第一人民医院接受MRI检查的老年健康受试者20例进行常规全脊柱MRI(T2WI、T1WI、T2 IDEAL)和IDEAL-IQ检查,对C2~L5椎体进行质子密度脂肪分数(PDFF)的测量,每个椎体测量3次取平均值。比较老年男性和女性间椎体PDFF的差异。结果老年女性组全脊柱椎体骨髓的PDFF均高于老年男性组(均P<0.01)。老年男性组全脊柱椎体骨髓PDFF从C2~7、T1~6、T7~12至L1~5分别为0.51±0.06、0.43±0.02、0.47±0.03、0.58±0.03;老年女性组全脊柱椎体骨髓PDFF从C2~7、T1~6、T7~12至L1~5分别为0.64±0.05、0.62±0.02、0.66±0.02、0.75±0.03。老年女性组颈椎、胸椎和腰椎椎体骨髓PDFF均高于老年男性组(均P<0.05),老年人全脊柱椎体骨髓PDFF与年龄无相关性(均P>0.05)。结论IDEAL-IQ能定量评价老年人全脊柱椎体骨髓PDFF,老年人全脊柱椎体骨髓PDFF呈现明显的解剖学变异,老年女性的全脊柱椎体骨髓PDFF高于老年男性。Objective To quantitatively assess the full spine vertebral bone marrow fat in the elderly with least-squares estimation quantitation sequence(IDEAL-IQ)technique.Methods Twenty elderly patients(10 males and 10 females,with an average age of 71±6)were scanned by conventional full spinal MRI(T2WI,T1WI,T2 IDEAL)and echo asymmetry and least-squares estimation quantitation sequence(IDEAL-IQ).Two experienced observers performed the full spine evaluation on conventional MRIs,and measured the proton density fat fraction(PDFF)of C2~L5 vertebral bodies.Each vertebral body was measured for three times and took the average value.Compare the differences in PDFF of the vertebral bodies between the male and female groups.Results The spine bone marrow PDFFs of the elderly male group at C2~7,T1~6,T7~12 and L1~5 were 0.51±0.06,0.43±0.02,0.47±0.03 and 0.58±0.03,respectively.The spine bone marrow PDFFs of the elderly female group at C2~7,T1~6,T7~12 and L1~5 were 0.64±0.05,0.62±0.02,0.66±0.02 and 0.75±0.03,respectively.The spine bone marrow PDFFs of C2~7,T1~6,T7~12 and L1~5 in the elderly female group was higher than those of the elderly male group(all P<0.05).The spine bone marrow PDFF was not correlated with age.Conclusion IDEAL-IQ can quantitatively evaluate the PDFF of the full spine vertebral bone marrow in the elderly.The PDFF of the elderly spine vertebral bone marrow has significant anatomical variation.The spine bone marrow PDFF of elderly women is higher than that of elderly men.
