传统脊柱前路手术和微创入路脊柱手术治疗脊椎感染的效果比较

摘　　要：目的探讨传统脊柱前路手术和微创入路脊柱手术治疗脊椎感染的临床效果。方法选取我院2015年4月~2018年9月收治的60例脊椎感染患者,分为对照组和观察组,对照组采用传统脊柱前路手术进行治疗,而观察组采用微创入路脊柱手术进行治疗。统计分析两组患者手术时间、术中出血量、总引流量、引流管留置时间及卧床时间、VAS评分、Cobb角矫正度、丢失度、伤椎椎管侵占率下降值、ASIA改善等级。结果观察组的术中出血量、总引流量、引流管留置时间及卧床时间分别为(85.42±20.47)mL、(61.34±19.57)mL、(20.13±5.42)h、(7.48±1.24)d,明显低于对照组(P<0.05);术后观察组的VAS评分为(2.74±0.97)分,其下降程度明显高于对照组(P<0.05)。观察组患者的Cobb角矫正度、伤椎椎管侵占率下降值、ASIA改善等级均显著高于对照组(P<0.05),Cobb角丢失度显著低于对照组(P<0.05)。结论对脊椎感染患者进行治疗时,微创入路脊柱手术具有更好的治疗效果,手术方式简单,疗效高,安全性好,而且术中出血量少、住院时间短,值得在脊椎感染患者的治疗中推广。Objective To explore the clinical effects of traditional anterior spinal surgery and minimally invasive spinal surgery on spinal infections.Methods A total of 60 patients with spinal infections treated in our hospital from April 2015 to September 2018 were divided into the control group and the observation group.The control group was treated with traditional anterior spinal surgery,while the observation group was treated with minimally invasive spinal surgery.Statistical analyses of operation time,intraoperative blood loss,total drainage volume,drainage tube indwelling time and bed rest time,VAS score,correction and loss degrees of Cobb angle,decrease value of injured spinal canal invasion rate,ASIA improvement level were conducted.Results The intraoperative blood loss,total drainage volume,drainage tube indwelling time and bed rest time of the observation group were(85.42±20.47)mL,(61.34±19.57)mL,(20.13±5.42)h and(7.48±1.24)d,which were significantly lower than those of the control group(P<0.05).The postoperative VAS score of the observation group was(2.74±0.97)points,and its decrease was significantly higher than that of the control group(P<0.05).The correction degree of Cobb angle,decrease value of injured spinal canal invasion rate,and ASIA improvement level of the observation group were significantly higher than those of the control group(P<0.05).The loss degree of Cobb angle of the observation group was significantly lower than that of the control group(P<0.05).Conclusion Compared with traditional anterior spinal surgery,minimally invasive spinal surgery has a better treatment effect,simpler surgery method,higher efficacy,higher safety,less intraoperative blood loss and shorter hospital stay in treatment of spinal infections,which is worthy to be popularized.
