甩肩疗法联合序贯康复包治疗肱骨近端Neer三、四部分骨折的疗效观察

摘　　要：目的探讨甩肩疗法联合序贯康复包治疗肱骨近端Neer三、四部分骨折的临床疗效。方法自2017年10月~2019年7月回顾性分析本科收治的15例肱骨近端Neer三、四部分骨折患者,上述患者均采用甩肩疗法联合序贯康复包治疗,比较患者治疗前后的VAS评分、Neer肩关节功能评分,并分析其并发症情况。结果15例均获得随访,时间为1~1.5年,平均13个月;其中局部出现水疱2例,肩关节脱位1例,畸形愈合1例,创伤性关节炎1例,未出现肱骨头坏死和神经损伤现象。VAS评分从治疗前的(7.44±0.82)分减少至治疗后12个月的(0.74±0.42)分,肩关节功能Neer评分从治疗前的(24.21±7.25)分改善至治疗后12个月的(92.25±4.54)分,上述两项指标治疗前、后比较,差异有统计学意义(P<0.05)。结论甩肩疗法联合序贯康复包治疗肱骨近端Neer三、四部分骨折,可较好地恢复肩关节功能,可明显缩短住院时间,且费用低廉、无手术创伤,值得临床推广应用。Objective To investigate the clinical efficacy of shoulder swing combined with sequential rehabilitation package in the treatment of proximal humerus Neer fractures in partⅢandⅣ.Methods A total of 15 patients with proximal humerus NeerⅢandⅣfractures in our department from October 2017 to July 2019 were retrospectively analyzed.The patients were treated with shoulder swing therapy combined with sequential rehabilitation package.VAS scores and Neer shoulder joint function scores before and after treatment were compared and their complications were analyzed.Results All 15 patients were followed up for 1-1.5 years,with 13 months on average.There were 2 cases of local vesicles,1 case of dislocation of shoulder joint,1 case of malunion,1 case of traumatic arthritis.And no necrosis of humerus head or nerve injury occurred.VAS score decreased from(7.44±0.82)points to(0.74±0.42)points 12 months after treatment,and Neer score of shoulder joint function improved from(24.21±7.25)points to(92.25±4.54)points 12 months after treatment.There were statistically significant differences in the above indicators between the two indicators before and after treatment(P<0.05).Conclusion Shoulder swing therapy combined with sequential rehabilitation package for the treatment of proximal humerus Neer fractures inⅢandⅣparts can better restore the function of the shoulder,significantly shorten the length of hospital stay,and has low cost,without surgical trauma,which is worthy of clinical application.
