奥沙利铂致感音神经性耳聋和急性视神经炎

摘　　要：1例59岁男性患者因胃癌伴多发淋巴结转移接受SOX方案化疗:奥沙利铂注射液200 mg入5%葡萄糖注射液250 ml静脉滴注2 h(第1天),替吉奥60 mg口服、2次/d(第1~14 d);每3周为1个周期。在第3周期治疗期间,输注奥沙利铂1周后,患者突然出现耳聋及视物模糊症状。经耳鼻喉科医师会诊,诊断为突发感音神经性耳聋和急性视神经炎。考虑患者突发感音神经性耳聋和急性视神经炎与奥沙利铂相关。停止化疗,并给予神经营养药物及改善微循环治疗,1周后,患者视力逐渐恢复正常,但是耳聋症状持续存在。A 59-year-old male patient received SOX chemotherapy regimen(IV infusion of oxaliplatin injection 200 mg dissolved in 5%glucose injection 250 ml for 2 hours on the first day,oral tegafur,gimeracil and oteracil potassium 60 mg once daily on day 1-14,every 3 weeks was a cycle)for gastric cancer with multiple lymph nodes metastases.One week after the infusion of oxaliplatin during the third cycle,the patient suddenly developed deafness and blurred vision.The patient was diagnosed with sudden sensorineural deafness and acute optic neuritis after otorhinolaryngologist consultation.His sudden sensorineural deafness and acute optic neuritis were considered to be associated with oxaliplatin.Chemotherapy was discontinued and the patient was given neurotrophic and microcirculation-improving medications.One 1 week later,the patient′s vision gradually returned to normal,but the deafness persisted.
