摘　　要：目的结合计算机断层扫描(CT)影像学改变,明确床旁超声检测量视神经鞘管直径(ONSD)能否准确评估颅脑损伤患者颅内压变化。方法选取2019年3—9月收治的颅脑损伤患者60例,每例患者均测量ONSD值并行头颅CT检查,比较ONSD值与头颅CT检查结果,分析两者之间的阳性率的统计学意义及对两者进行一致性研究。结果超声测量ONSD值的阳性率为85%,CT显示阳性率为75%,通过配对卡方检验结果认为超声测量ONSD与CT评估颅内压增高两者检验阳性率比较差异无统计学意义(P>0.05),两者进行一致性检验说明两者检查结果一致性较好。结论床旁超声测量视神经鞘管直径可用于评估成人颅脑损伤患者颅内压变化。Objective Combined with CT imaging changes,it was determined whether the optic nerve sheath diameter(ONSD)detected by bedside ultrasound could accurately evaluate the intracranial pressure change in patients with craniocerebral injury.Methods A total of 60 patients with craniocerebral injury admitted from March 2019 to September 2019 were selected.ONSD value was measured and head CT was performed in each patient.The results of ONSD value and head CT were compared to analyze the statistical significance of the positive rate between the two and the consistency between the two.Results The positive rate of ONSD value by ultrasonic measurement was 85%,and the positive rate of CT was 75%.According to the paired chi-square test results,there was no statistically significant difference in the positive rate of ONSD by ultrasonic measurement and increased intracranial pressure by CT evaluation,and the consistency test between the two indicated that the consistency of the two test results was good.Conclusion The optic nerve sheath diameter measured by bedside ultrasound can be used to evaluate the change of intracranial pressure in adult patients with craniocerebral injury.
