摘　　要：目的探索显微镜下血管鞘膜内分离技术在精索静脉结扎术中的可行性和安全性。方法67例精索静脉曲张患者均采用了显微镜下血管鞘膜内分离技术用于游离结扎精索静脉,记录手术时间、术中游离出来的静脉数目、动脉数目,统计术后并发症发生例数,统计患者术后精液改善百分率。结果67例患者都顺利完成手术,平均手术时间42.8 min,游离静脉9.2条,侧支3.6条,保留动脉1.1条,术后没有并发睾丸萎缩、鞘膜积液等病例,没有复发病例,精液改善率是85.1%。结论显微镜下血管鞘膜内分离技术在精索静脉结扎术中应用是安全可行及有效的,可降低手术难度,保护动脉,降低手术并发症。Objactive To explore the safety and feasibility of the application of vascular intrathecal approach for microsurgical varicocelectomy.Methods The 67 patients underwent microsurgical varicocelectomy through the vascular intrathecal approach.The operation time,the number of vein and artery,the postoperative complications,and the percent of the improvement of postoperative quality of the sperm were recorded.Results The 67 patients were performed successfully.The average operation time was 42.8 min,the numbers of the veins and arteries were 9.2 and 1.1,the number of the branch veins was 3.6,there were no complications such as orchiatrophy or hydrocele of tunica vaginalis,no recurrence,the percent of the improvement of quality of the sperm was 85.1%.Conclusion The application of the vascular intrathecal approach for microsurgical varicocelectomy is safe and feasible.
