腹腔镜完全腹膜外疝修补术与开放疝修补术在圆韧带囊肿合并斜疝患者中的应用

摘　　要：目的探讨腹腔镜完全腹膜外疝修补术(TEP)与开放疝修补术在圆韧带囊肿合并斜疝患者中的应用。方法选择2016年1月至2019年1月,浙江省人民医院腹股沟疝合并子宫圆韧带囊肿患者50例作为研究对象,随机分为对照组(25例)和试验组(25例),其中对照组患者行开放无张力腹股沟疝修补术,试验组患者行TEP,分析2组患者治疗后的手术相关指标、并发症发生率和复发率。结果试验组患者的手术时间(44.01±6.66)min和对照组患者的手术时间(48.31±9.32)min,差异无统计学意义(P>0.05)。试验组患者住院时间(3.13±1.01)d和术中出血量(5.33±2.04)ml明显少于对照组(18.74±0.52)d和术中出血量(11.17±0.41)ml,差异均有统计学意义(P<0.05)。试验组患者的并发症发生率及复发率均低于对照组,差异均有统计学意义(P<0.05)。结论对腹股沟疝合并子宫圆韧带囊肿患者实施TEP,患者术中相关指标较优,且不良反应和复发率均低,临床治疗中推广和应用。Objective To investigate the effect of laparoscopic total extraperitoneal hernia repair(TEP)and open hernia repair in patients with inguinal hernia complicated with uterine round ligament cyst.Methods From January 2016 to January 2019,50 patients with inguinal hernia complicated with uterine round ligament cyst were selected as the research objects.According to their admission sequence,they were divided into control group(n=25)and experimental group(n=25).Patients in control group underwent open tension-free inguinal hernia repair,while patients in experimental group underwent TEP repair.After treatment,the operation related indicators,the incidence of complications and recurrence rate of the two groups were analyzed.Results There was no significant difference in the operation time between the experimental group and the control group[(44.01±6.66)min vs(48.31±9.32 min,P>0.05)].The hospitalization time and intraoperative blood loss of the experimental group(3.13±1.01 d,5.33±2.04 ml)were significantly less than those of the control group(18.74±0.52 d,11.17±0.41 ml);The incidence of complications and recurrence in the experimental group were significantly lower than those in the control group,with statistical significance(P<0.05).Conclusion The intraoperative related indicators in TEP repair for inguinal hernia complicated with uterine round ligament cyst is better,and the adverse reaction and recurrence rate are low.TEP repair is worthy of promotion and application in clinical treatment.
