酚红棉线试验与Schirmer试验在干眼诊断中的比较研究

摘　　要：目的比较酚红棉线试验和Schirmer试验的重复性、在干眼诊断上的灵敏度和特异度、两者的一致性等。方法分别收集48例干眼患者和12例正常人,依次进行干眼问卷调查、裂隙灯体征检查、酚红棉线试验、Schirmer试验及2种试验的刺激性调查,并选择其中12例干眼患者进行2种泪液分泌试验的一致性检验。对数据进行统计学分析检验。结果酚红棉线试验和Schirmer试验均有一定的可重复性;对干眼诊断的敏感度分别为44.2%和53.5%,特异度分别为81.8%和54.5%;2种试验的一致性kappa值为0.229,而酚红棉线试验的刺激性显著低于Schirmer试验(P<0.05)。结论酚红棉线试验和Schirmer试验均有一定的可重复性,但两者对于干眼诊断的灵敏度均不高,且一致性较差,因此均只能作为诊断干眼的辅助手段,而酚红棉线试验相较Schirmer试验具有较高的特异度和舒适程度,可以帮助医师更好地做出排除诊断。Objective To compare the repeatability and agreement of phenol red thread(PRT)test and Schirmer test,as well as the sensitivity and specificity in diagnosing dry eyes.Methods Forty-eight patients with dry eye and 12 normal people were collected.Dry eye questionnaires,slit lamp examinations,PRT test,Schirmer test and irritation score to two tests were completed in sequence.In addition,12 dry eye patients accepted the second and the third tests of PRT test and Schirmer test for the exploration of repeatability.All the statistics were analyzed by SPSS software.Results Both the PRT test and Schirmer test were repeatable in this study.The sensitivity of PRT test and Schirmer test for diagnosing dry eye was 44.2%and 53.5%respectively,while the specificity was 81.8%and 54.5%.Furthermore,the kappa value was only 0.229 for the two tests.However,the irritation score of PRT test was dramatically lower than that of Schirmer test.Conclusions Both PRT test and Schirmer test were repeatable to some extent,but they had poor sensitivity and agreement,making them preferably be the supplementary methods for diagnosing dry eye.Compared with Schirmer test,the high specificity and low irritation of PRT test may give doctors a better choice to exclude a dry eye.
