摘　　要：目的通过了解某肿瘤专科医院静脉配置中心退药情况,促进临床科室正确开医嘱与合理用药,加强药品质量管理提供参考。方法收集该院2018年7月~2019年6月退药申请5713帖,按退药原因,按退药科室,退药品种3方面统计退药帖数、退药金额及其所占的比率进行回顾性分析。结果退药帖数最多及退药金额最大的前3项原因依次是修改医嘱、病情变化、未写原因。而申请退药帖数最多的前3个科室依次为内科、放疗科、鼻咽科,退药金额最大前3个科室依次为内科、放疗科、综合科。退药帖数及金额最多的药品为抗代谢类抗肿瘤药、铂类抗肿瘤药、植物类抗肿瘤药。结论静脉配置中心应加强退药管理,加强医患沟通,加强合理用药,减少退药。OBJECTIVE To understandthe specific circumstances of drug refunds in PIVAS of specialist hospitaland to promote the clinical departments to use drugreasonably and provide reference for strengthening the quality control of drugs.METHODS 5713 applications for drug refund by the PIVAS of specialist hospital in July 2018 to June 2019 were collected.After sorted through the reasons for drug refund,the variety of refund and the departments of drug refund,the types of drugs refunded,the sum of refunds and the ratio of refunds were analyzed retrospectively.RESULTS The reason of the largest of quantity and proportion for drug refund and the largest amount were from doctor’s advicerevise,patients’condition change,the writings without reason.The largest amounts with drug refunds departmentwere internal medicinet,radiotherapy and nasopharyngolog.The largest quantity with drug refund were internal medicine,radiotherapy,and integrated branch.Drugs with the largest number and amount of drug refunds were anti-metabolism antitumor drugs,platinum anti-tumor drugs and plant anti-tumor drugs.CONCLUSION The PIVAS should strengthen the management of drug refund,strengthen the communication between doctors and patients,strengthen drug userationally,and reduce drug refund.
