营养控制状态评分与老年血液透析病人生存状态的相关性探讨

摘　　要：目的探究营养控制状态(CONUT)评分与老年血液透析病人生存状态的相关性。方法对我院2015~2019年维持性血液透析的老年(年龄≥60岁)病人进行回顾性分析,采用Spearman相关系数及COX回归分析CONUT评分和老年血液透析病人生存状态的相关性。结果CRP与CONUT评分呈正相关;BMI、Hb、TC、TG、白蛋白、前白蛋白与CONUT评分呈负相关。与重度营养不良的老年血液透析病人相比,轻中度营养不良的老年血液透析病人的死亡风险显著降低(HR=0.378,95%CI:0.155~0.923,P=0.033)。结论CONUT评分评估的营养状态与老年血液透析病人死亡风险存在一定的相关性。Objective To explore the correlation between Controlling Nutritional Status(CONUT)score and survival status of the elderly patients with hemodialysis.Methods The elderly patients(age≥60 years old)received maintenance hemodialysis were collected from January 2015 to December 2019 in our hospital.The association between survival status of elderly hemodialysis patients and CONUT score were retrospective analyzed by Spearman correlation coefficient and COX regression.Results C reaction protien was positively correlated with CONUT score,whereas body mass index,hemoglobin,total cholesterol,triglyceride,albumin and prealbumin were negatively correlated with the CONUT score.Compared with elderly hemodialysis patients in severe malnutrition condition,the risk of death in those with mild to moderate malnutrition was significantly reduced(HR=0.378,95%CI:0.155-0.923,P=0.033).Conclusions The nutritional status assessed by CONUT score is correlated with the mortality risk of elderly patients with maintenance hemodialysis.
