摘 要:目的调查石家庄地区事业单位中老年人群高同型半胱氨酸血症(HHcy)以及代谢综合征(metabolic syndrome,MS)的患病状况,分析血清Hcy水平与MS的关系。方法本研究采用整群抽样的方法,抽取河北省人民医院体检中心单位团体查体的20个事业单位40岁以上人群,共计2572人纳入本次横断面调查研究,根据年龄分为2组,<60岁人群为非老年组(1502人),≥60岁人群为老年组(1070人)。血清Hcy水平的影响因素分析采用Pearson相关分析;采用Logistic回归分析血清Hcy水平与MS的关系。结果(1)在老年组和非老年组中,HHcy的患病率分别为35.11%和22.84%,MS的患病率分别为45.67%和37.32%。(2)校正年龄、性别和血肌酐水平后的相关性分析结果显示,2组人群中影响血清Hcy水平的主要影响因素均为腰围、TG、SBP、DBP、HDL-C、UA。(3)根据血清Hcy水平四分位数由低到高将2组人群分别分成Q1~Q4亚组,Logistic回归结果显示,老年组和非老年组中Q4亚组患MS的风险是Q1亚组的2.68、1.40倍(均P<0.05);2组人群中,发生MS的风险均随血清Hcy水平的升高而增加;且老年组人群中Q4亚组发生MS各组分的风险大于非老年组人群Q4亚组。结论血清Hcy水平升高的人群发生MS的风险增大,且老年组的风险较非老年组更大。Objective To investigate the incidence of metabolic syndrome(MS)and hyperhomocysteinemia(HHcy)in middle-aged and aged people of state-run institutions,and to analyze the relationship between MS and serum homocysteine(Hcy)level.Methods A total of 2572 subjects were enrolled in this cross-sectional study and divided into two groups,aged group(aged≥60,n=1070)and non-aged group(40≤aged<60,n=1502).The influencing factors of serum Hcy level were analyzed by Pearson correlation analysis.The relationship between serum Hcy level and MS was analyzed by Logistic regression analysis.Results(1)The incidence rate of HHcy was 35.11%in aged group and 22.84%in non-aged group,and the incidence rate of MS was 45.67%and 37.32%in the two groups respectively.(2)After adjusting for age,sex and serum creatinine level,Pearson correlation analysis showed that waist circumference,triglycerides(TG),systolic blood pressure(SBP),diastolic blood pressure(DBP),high density lipoprotein cholesterol(HDL-C)and uric acid(UA)were significantly associated with Hcy level in the two groups.(3)According to the serum Hcy level,subjects in the two groups were divided into Q1~Q4 subgroup.Logistic regression analysis showed that the subjects in the highest Hcy quartile(Q4 subgroup)had 2.68-fold,and 1.40-fold increased risk of MS in aged group and non-aged group respectively.The risk of each component of MS in aged Q4 subgroup was higher than that in non-aged Q4 subgroup.Conclusions Individuals with higher Hcy level were more likely to develop MS and its components,especially in aged subjects.
