摘　　要：目的分析负压封闭引流(VSD)联合桡动脉掌浅支腕横纹游离皮瓣对手部创伤的修复效果及其对手功能的影响。方法选取我院收治的68例手部创伤患者作为研究对象,随机将其分为对照组(34例,常规引流术+桡动脉掌浅支腕横纹游离皮瓣)和观察组(34例,VSD+桡动脉掌浅支腕横纹游离皮瓣)。比较两组的修复效果、手功能恢复情况、血清炎性因子水平及瘢痕评分。结果观察组的修复优良率、手功能恢复优良率均显著高于对照组(P<0.05)。治疗后7 d,两组CRP、IL-8、PCT水平均下降,且观察组显著低于对照组(P<0.05)。治疗后6个月,两组的瘢痕评分均降低,且观察组显著低于对照组(P<0.05)。结论VSD联合桡动脉掌浅支腕横纹游离皮瓣对手部创伤后的修复效果显著,可降低患者血清炎性因子水平,减轻瘢痕生长,促进手功能恢复,值得临床推广应用。Objective To analyze the repair effect of vacuum sealing drainage(VSD)combined with transverse free flap of wrist with superficial palmar branch of radial artery on hand trauma and its influence on hand function.Methods A total of 68 patients with hand trauma admitted in our hospital were selected as the research objects and randomly divided into control group(34 cases,routine drainage+transverse free flap of wrist with superficial palmar branch of radial artery)and observation group(34 cases,VSD+transverse free flap of wrist with superficial palmar branch of radial artery).The repair effect,hand function recovery,serum inflammatory factors levels and scar score were compared between the two groups.Results The excellent and good rate of repair and hand function recovery in the observation group were significantly higher than those in the control group(P<0.05).At 7 d after treatment,the levels of CRP,IL-8 and PCT in the two groups decreased,and those in the observation group were significantly lower than the control group(P<0.05).At 6 months after treatment,the scar scores of the two groups decreased,and that in the observation group was significantly lower than the control group(P<0.05).Conclusion VSD combined with transverse free flap of wrist with superficial palmar branch of radial artery has a significant repair effect on the hand trauma,it can reduce the serum inflammatory factors levels and scar growth,and promote hand function recovery,which is worthy of clinical promotion and application.
