摘 要:目的探讨四维超声时间-空间关联成像技术(STIC)在胎儿心脏畸形辅助诊断中的应用价值。方法选择2018年10月至2019年10月疑似存在胎儿心脏畸形的68例孕妇为研究对象,均给予二维超声心动图和四维超声STIC检查。以尸检病理诊断为金标准,比较两种检查方法对胎儿心脏切面的显示合格率、所用时间及诊断效能。结果68例疑似存在胎儿心脏畸形的孕妇中,56例经引产后尸检病理诊断确诊。四维STIC对左、右室流出道和主动脉弓的显示合格率高于二维超声(P<0.05)。四维STIC检查每例样本及每幅图像/容积所用时间均短于二维超声检查(P<0.05)。四维STIC诊断胎儿心脏畸形的灵敏度、特异度、准确度均高于二维超声(P<0.05)。结论利用四维STIC诊断胎儿心脏畸形可清晰显示胎儿心脏各标准切面,检查速度快,诊断效能高。Objective To explore the application value of four-dimensional ultrasound time-space correlation imaging technology(STIC)in the auxiliary diagnosis of fetal heart malformation.Methods Sixty-eight pregnant women with suspected fetal heart malformations from October 2018 to October 2019 were selected as the research objects and given two-dimensional echocardiography and four-dimensional ultrasound STIC examination.Taking autopsy pathological diagnosis as the gold standard,the qualified rate of displaying fetal heart section,time and diagnostic efficiency of the two examination methods were compared.Results Among 68 pregnant women with suspected fetal heart malformation,56 cases were confirmed by pathological diagnosis after induced labor autopsy.The qualified rates of four-dimensional ultrasound STIC for displaying left,right ventricular outflow tract and aortic arch were higher than those of two-dimensional ultrasound(P<0.05).The time of examining each sample and each image/volume by four-dimensional ultrasound STIC were shorter than those by two-dimensional ultrasound(P<0.05).The sensitivity,specificity and accuracy of four-dimensional ultrasound STIC in the diagnosis of fetal heart malformation were higher than those of two-dimensional ultrasound(P<0.05).Conclusion The four-dimensional ultrasound STIC in the diagnosis of fetal heart malformation can clearly display all standard sections of fetal heart,with fast examination speed and high diagnostic efficiency.
