超声引导下关节腔药物注射联合独活寄生汤治疗强直性脊柱炎髋关节受累的疗效评价

摘　　要：目的:观察超声引导下关节腔药物注射联合独活寄生汤治疗强直性脊柱炎(AS)髋关节受累的临床疗效。方法:选取56例髋关节受累的AS患者,给予超声引导下关节腔药物注射联合独活寄生汤治疗,分别从功能活动、视觉模拟评分(VAS)、髋关节Harris评分及安全性评估等方面评定临床疗效。结果:与治疗前比较,56例治疗后3个月患者髋关节功能活动度、VAS评分和髋关节Harris评分均有明显改善,差异均有统计学意义(均P<0.05);治疗后3个月红细胞沉降率(ESR)及C反应蛋白(CRP)有不同程度变化,差异均有统计学意义(均P<0.05);治疗后3个月肝功及肾功相关指标无明显变化,差异均无统计学意义(均P>0.05)。结论:超声引导下关节腔药物注射联合独活寄生汤治疗可有效缓解疼痛、改善关节功能,是治疗AS髋关节受累的有效方法。Objective:To observe clinical efficacy of ultrasound-guided intra-articular injection combined with Duhuo Jisheng Decoction in the treatment of ankylosing spondylitis(AS)with the hip joint disease.Methods:56 out-patients who had AS with the hip joint disease were studied,who were performed intra-articular injection by the ultrasound guided combined with Duhuo Jisheng Decoction.Each patient was evaluated from the following three aspects:the clinical manifestation,the visual analogue scale(VAS),the hip Harris scores and safety evaluations.Results:Compared to before treatment,the range of motion,VAS and Harris scores were significantly different at 3 months after treatment(all P<0.05),the value of ESR and CRP changed significantly at 3 months after treatment(both P<0.05),the related changes with liver function and renal function had no obvious improvement at 3 months after treatment(all P>0.05).Conclusions:Ultrasound guided joint cavity drug injection combined with Duhuo Jisheng Decoction could effectively relieve pain and improve joint function,which is an effective measurement for the treatment of AS hip joint involvement.
