摘 要:目的探讨新型冠状病毒肺炎疫情期间甲状腺结节择期手术患者心理状况及影响因素。方法选取2020年2—4月中南大学湘雅医院收治的82例甲状腺结节手术患者作为研究对象,应用一般资料调查问卷、广泛性焦虑量表(GAD-7)、患者健康问卷抑郁症状群量表(PHQ-9)、新型冠状病毒肺炎知识调查问卷、社会支持量表对患者进行调查,采用多元回归分析影响甲状腺结节择期手术患者心理状态的相关因素。结果新型冠状病毒肺炎疫情期间甲状腺结节患者GAD-7评分为(16.85±3.78)分,PHQ-9评分为(21.02±4.02)分。经Logistic多元回归分析可知,新型冠状病毒肺炎认识不足是导致甲状腺结节患者焦虑的主要影响因素,年龄、学历是导致甲状腺结节患者抑郁的主要影响因素,而社会支持中客观支持、支持利用度、接受新型冠状病毒肺炎防护宣教是新型冠状病毒肺炎疫情期间甲状腺结节患者焦虑、抑郁的保护因素。结论新型冠状病毒肺炎疫情期间甲状腺结节择期手术患者焦虑、抑郁情绪较明显,对患者进行新型冠状病毒肺炎相关知识宣教,给予患者社会支持有助于患者积极面对疾病,减轻新型冠状病毒肺炎疫情期间甲状腺结节手术患者术前不良情绪,有利于促进患者身心健康。Objective To explore the psychological status and influencing factors of patients with thyroid nodule surgery during the outbreak of coronavirus disease(COVID-19). Methods A total of eighty-two patients with thyroid nodule admitted to Xiangya Hospital, Central South University were selected as the research objects and was surveyed with general information questionnaire, the generalized anxiety scale(GAD-7) score, the healthy and stable depression symptom scale(PHQ-9), the coronary disease knowledge questionnaire, social support scale etc. Related factors affecting the mental status of patients undergoing elective surgery for thyroid nodules were analyzed with multiple regression analysis. Results The GAD-7 score of patients with thyroid nodules was(16.85±3.78) points and PHQ-9 score was(21.02±4.02) points during COVID-19 epidemic. Logistic multiple regression analysis showed that patients’ lack of knowledge about COVID-19 were the main factors that caused anxiety in patients with thyroid nodules. Age and education were the main factors that caused depression in patients with thyroid nodules. In social support, objective support, support utilization, and acceptance of COVID-19 prevention and education were protective factors for anxiety and depression in patients with thyroid nodules during the outbreak of COVID-19. Conclusion Patients with thyroid nodule undergoing elective surgery during COVID-19 epidemic have more anxiety and depression. Educating patients about COVID-19 related knowledge and providing social support to patients will help patients actively face the disease and relieve unhealthy emotions of patients with thyroid nodule surgery COVID-19 epidemic. And it is conducive to patients’ physical and mental health.
