摘 要:新月体肾炎在临床上往往表现为急进性肾炎综合征,是肾脏病领域常见的急危重症,其病理核心为包曼囊内超过50%肾小球新月体形成,且新月体占肾小球面积50%以上。新月体可分为细胞性、细胞纤维性和纤维性新月体,疾病早期以细胞性新月体形成为主。一般认为足细胞并不参与新月体形成,但近十年来临床试验和动物实验研究提示足细胞在新月体肾炎病理因子刺激下可能发生细胞表型转化,从而获得迁移能力并形成足细胞桥,最终启动新月体形成的病理进程。本文就足细胞在新月体肾炎中的相关研究进展作一综述。Crescentic glomerulonephritis(GN)clinically manifests as acute glomerulonephritis syndrome,and is a common acute critical disease in the area of kidney diseases.Its pathology is characterized by formation of more than 50%of glomerular crescents in the Bowman's capsule,accounting for over 50%of the glomerular area.At the early stage of disease,cellular crescents are very common.Three types of crescents have been identified as celluar,brocellular and fibrous crescents.The etiology and the initial pathogenetic factors are different in the three types.It is believed generally that podocytes are not involved in crescent formation,but clinical and animal studies in the past decade have suggested that podocytes may undergo cell phenotypic transformation after the stimulation of pathological factors of crescentic glomerulonephritis,thereby obtaining migration ability and forming a podocyte bridge.The pathological process of crescent formation is finally initiated.The research progress of podocytes in crescentic glomerulonephritis is reviewed in this article.
