摘  要:目的探讨23S rRNA突变和ermB基因与解脲脲原体(Uu)对大环内酯类抗生素耐药的相关性。方法选取2017年12月至2019年2月经培养及核酸检测确认,并经药敏实验筛选的90例Uu样本进行研究,分为敏感组、中敏组、耐药组,每组30例。比较3组间23S rRNA突变率、突变位点及ermB阳性率的差异,分析23S rRNA突变与ermB及耐药表型间的关系。结果共发现23SrRNA突变样本18例,其中13例突变类型为G2232T,该突变在3组中均存在;其他5例仅存在于中敏组和/或耐药组,包含C2409T、G2143T、C2518G 3种突变类型。ermB在3组中阳性率分别为20.00%、66.67%、70.00%,耐药组、中敏组明显高于敏感组(x^2=13.303,P<0.01;x^2=15.152,P<0.01);此外,18例23S rRNA突变样本中伴有ermB阳性17例。结论 23S rRNA突变率较低,与Uu对大环内酯类抗生素耐药的相关性较弱;ermB阳性率很高,可能在介导Uu对大环内酯类抗生素耐药的机制中起主要作用。Objective To investigate the relationship between 23S rRNA mutation and ermB gene with Ureaplasma urealyticum(Uu)resistance to macrolide antibiotics.Methods 90 Uu samples were selected from December 2017 to February 2019,which were confirmed by culture and nucleic acid detection,and screened by drug sensitivity test.They were divided into sensitive group,middle sensitive group and drug resistant group,with 30 cases in each group.The difference between 23S rRNA mutation rate,mutation site and ermB positive rate among the three groups were compared,and the relationship between 23S rRNA mutation,ermB and drug resistance phenotype was analyzed.Results A total of 18 mutant samples were detected and G2232T was found in 13 of these samples which was present in all three groups.The other 5 samples included C2409T,G2143T and C2518G,which existed only in the middle sensitive group and/or drug resistant group.The positive rates of ermB in the three groups were 20.00%,66.67% and 70% respectively.The drug-resistant group and the middle-sensitive group were significantly higher than the sensitive group.In addition,17 of the 1823S rRNA mutant samples were positive for ermB.Conclusion The low mutation rate of 23S rRNA showed a weak correlation with the resistance of Uu to macrolide antibiotics,and the high positive rate of ermB suggested that it might play a major role in the mechanism of Uu resistance to macrolide antibiotics.
