摘  要:目的探讨肺外结核患者应用γ干扰素释放试验(IGRAs)与结核菌素试验(PPD试验)的应用价值差异。方法选取本院2016年6月至2019年6月就诊的肺外结核疑似患者149例作为研究对象,分别采用PPD试验及IGRAs,比较两种检测方法对于肺外结核的诊断效能。结果IGRAs试验与PPD试验结果比较,对于肺外结核患者的诊断结果差异有统计学意义(P<0.05)。IGRAs试验的灵敏度为86.24%、特异性为95.00%、阳性预测值为97.92%、阴性预测值为71.70%、约登指数为0.812。IGRAs试验的ROC曲线下面积为0.906,显著高于PPD试验的ROC曲线下面积0.562(P<0.01)。结论IGRAs用于肺外结核诊断中的效能优于PPD试验,针对可疑肺外结核患者可以γ干扰素释放试验为主要依据,尤其是避免误诊的临床价值更高。Objective To investigate the difference of application value between interferon gamma release test(IGRAs)and tuberculosis test in patients with extrapulmonary tuberculosis.Methods From June 2016 to June 2019,149 suspected patients with extrapulmonary tuberculosis were selected as the main subjects.PPD test and IGRAs were used to compare the diagnostic efficacy of the two methods for extrapulmonary tuberculosis.Results There was significant difference between IGRAs test and PPD test in the diagnosis of extrapulmonary tuberculosis(P<0.05).The sensitivity,specificity,positive predictive value,negative predictive value and Jordan index of IGRAs were 86.24%,95.00%,97.92%,71.70% and 0.812,respectively.The area under ROC curve in IGRAs test was 0.906,which was significantly higher than that under ROC curve of PPD test(0.562)(P<0.01).Conclusion The efficacy of IGRAs in the diagnosis of extrapulmonary tuberculosis is better than that of PPD test.The IGRAs is the main basis for suspected extrapulmonary tuberculosis,especially the clinical value of avoiding misdiagnosis is higher.
