阴道彩色多普勒超声与MRI诊断剖宫产切口妊娠的价值探析

摘　　要：目的探析阴道彩色多普勒超声与磁共振成像(MRI)诊断剖宫产切口妊娠(CSP)的价值。方法选取2016年5月至2019年5月于本院检查的疑似CSP患者90例,所有患者进行阴道彩色多普勒超声与MRI检查,比较两种方法的诊断效能。结果90例疑似CSP患者中病理确诊为CSP患者75例,阴道彩色多普勒超声检查诊断为CSP患者61例(其中2例误诊),诊断为宫内早孕29例(其中漏诊16例);MRI检查诊断为CSP患者73例(其中2例误诊),诊断为宫内早孕17例(其中漏诊4例),MRI诊断CSP的准确性、敏感度、阴性预测值显著高于阴道彩色多普勒超声诊断;MRI诊断CSP的AUC为0.973(95%CI:0.915~0.996)显著优于阴道彩色多普勒超声诊断CSP的0.893(95%CI:0.811~0.949)。结论与阴道彩色多普勒超声比较,MRI诊断CSP的准确性更高,误诊、漏诊率更低,具有更高的临床诊断价值。Objective To explore the value of transvaginal color Doppler ultrasound and magnetic resonance imaging(MRI)in the diagnosis of incisional pregnancy in cesarean section.Methods A total of 90 patients with suspected CSP who were examined in our hospital from May 2016 to May 2019 were selected.All patients were examined by transvaginal color Doppler ultrasonography and MRI,and the diagnostic efficacy of the two methods was compared.Results Among 90 patients suspected of CSP,75 cases were pathologically diagnosed as CSP,61 cases were diagnosed as CSP by transvaginal color Doppler ultrasonography(2 cases were misdiagnosed),and 29 cases were diagnosed as intrauterine early pregnancy(16 cases were missed).73 cases of CSP were diagnosed by MRI(2 cases were misdiagnosed)and 17 cases were diagnosed as early intrauterine pregnancy(4 cases were missed).The accuracy,sensitivity and negative predictive value of CSP diagnosed by MRI were significantly higher than those diagnosed by transvaginal color Doppler ultrasound.The AUC of CSP diagnosed by MRI was 0.973(95%CI:0.915-0.996),which was significantly better than that of 0.893(95%CI:0.811-0.949)diagnosed by transvaginal color Doppler ultrasound.Conclusion Compared with transvaginal color Doppler ultrasound,MRI has higher accuracy in the diagnosis of CSP,lower rate of misdiagnosis and missed diagnosis,and has better clinical diagnostic value.
