摘 要:目的探讨miR-155及超敏C-反应蛋白(hs-CRP)在新生儿早期感染中的诊断价值。方法选择2017年5月-2019年6月山东省滕州市中心人民医院新生儿科收治的新生儿早期感染患儿85例为研究组,并结合临床表现及实验室检测结果分为细菌感染组(n=46例)和病毒感染组(n=39例);选择同期健康体检新生儿48名为对照组。采用免疫比浊法测定hs-CRP水平;采用实时荧光聚合酶链反应(PCR)测定miR-155水平;绘制受试者工作特征曲线(ROC),分析miR-155联合hs-CRP在新生儿早期感染中的诊断效能。结果研究组新生儿早期感染患儿miR-155水平低于对照组(P<0.05),hs-CRP水平均高于对照组(P<0.05);细菌感染组miR-155水平与病毒感染组比较无统计学意义,hs-CRP水平高于病毒感染组(P<0.05);ROC曲线结果表明:miR-155联合hs-CRP在新生儿早期感染中敏感性、特异性及准确率均高于单一miR-155、hs-CRP(P<0.05)。结论miR-155、hs-CRP在新生儿早期感染中表达异常,能实现细菌感染、病毒感染鉴别,且二者联合检测用于新生儿早期感染中能获得较高的诊断效能。OBJECTIVE To explore the value of miR-155and high-sensitivity C-reactive protein(hs-CRP)in diagnosis of early infection in neonates.METHODS Totally 85neonates with early infection who were treated in neonatology department of Tengzhou Central People′s Hospital from May 2017to Jun.2019were chosen as the study group and were divided into the bacterial infection group with 46cases and the viral infection group with 39cases according to the clinical manifestations and result of clinical laboratory test,and 48healthy neonates who received physical examination were set as the control group.The hs-CRP level was determined by using immunoturbidimetry,the miR-155level was determined by means of real-time fluorescent polymerase-chain-reaction(PCR),the receiver-operating-characteristic(ROC)curve was drawn to analyze the efficiency of miR-155combined with hs-CRP in diagnosis of early infection in the neonates.RESULTS The miR-155level of the study group was significently lower than that of the control group(P<0.05),the hs-CRP level of the study group was significently higher than that of the control group(P<0.05).There was no significant difference in the miR-155level between the bacterial infection group and the viral infection group,and the hs-CRP level of the bacterial infection group was significently higher than that of the viral infection group(P<0.05).ROC curve analysis indicated that the sensitivity,specificity and accuracy of the miR-155combined with hs-CRP were significently higher than those of the single miR-155or hs-CRP(P<0.05).CONCLUSIONThe neonates with early infection show abnormal expression of miR-155and hs-CRP,which may facilitate the differential diagnosis of bacterial infection and viral infection,and the joint detection of the two indexes can achieve high efficiency in diagnosis of early infection in the neonates.
