江西省南昌市某社区以妇科检查为切入点的低档暗娼综合干预模式效果评估

摘　　要：目的探讨以妇科检查为切入点的低档暗娼综合干预模式,根据低档暗娼的艾滋病相关知识、态度和行为的变化情况、性病患病及就诊情况等,了解该模式在实际运用过程中的成效。方法运用横断面调查方法,采用统一调查问卷,干预前对该社区的所有低档暗娼进行基线调查,干预过程中施以妇科检查,同时对她们采取艾滋病综合干预,干预结束后再次对该人群进行调查以评价干预效果。结果基线调查111人,终线调查106人。实施干预后,低档暗娼艾滋病防治知识总知晓率由47.7%上升到78.6%,最近1次商业性行为中安全套使用比例由75.7%上升到83.5%;最近1个月商业性行为坚持每次都用安全套的比例由56.8%上升到73.8%,性病患病率下降,主动求医行为增多。结论以妇科检查为切入点综合干预模式有效提高了低档暗娼的艾滋病防治知识知晓率、性行为中安全套的使用率,主动求医意识增强,这些行为有利于降低艾滋病的发病和传播。Objective To explore the comprehensive intervention model of low-grade prostitutes based on gynecological examination,and to understand the effectiveness of the model in the actual application process according to the changes of AIDS-related knowledge,attitude and behavior of low-grade prostitutes,venereal diseases and medical treatment.Methods A cross-sectional survey and a unified questionnaire were used to conduct a baseline survey of all low-grade prostitutes in the community before the intervention.Gynecological examinations were conducted during the intervention,and comprehensive AIDS interventions were conducted among them.After the intervention,the population was surveyed again to evaluate the intervention effect.Results There were 111 baseline surveys and 106 final surveys.After the intervention,the awareness rate of AIDS prevention knowledge among low-grade prostitutes increased from 47.7%to 78.6%,the condom use rate in the last commercial sexual behavior increased from 75.7%to 83.5%,and the rate of insisting on using condom every time in the last month increased from 56.8%to 73.8%,the incidence of STDs decreased,and the voluntary medical seeking behavior increased.Conclusions The comprehensive intervention model with gynecological examination as the entry point effectively improves the awareness rate of AIDS prevention knowledge among low-grade prostitutes,condom use rate in sexual behavior,and the awareness of seeking medical treatment on their own initiative.These behaviors are conducive to reducing the incidence and transmission of AIDS.
