摘 要:目的 探讨多孔硅胶管联合猪尾导管与常规硅胶管对胸外科手术后的引流效果.方法 前瞻性纳入2018-01—2019-10间于珠海市第五人民医院及中国人民解放军联勤保障部队第九四〇医院行胸腔镜手术治疗的患者.按应用的引流管分为多孔硅胶管联合猪尾导管组(联合组)和常规硅胶管组(常规组).比较2组患者的一般资料,术后12 h、24 h、48 h、72 h的疼痛程度,以及术后24 h引流量、总引流量、拔管时间、胸腔穿刺术情况和住院时间.结果 共纳入260例患者,每组130例.2组患者的一般资料及术后总引流量差异无统计学意义(P>0.05).联合组患者术后12 h、24 h、48 h、72 h疼痛程度,术后24 h引流量、拔管时间、术后行再次胸穿例数及次数,以及术后住院时间等指标,均优于常规组,差异有统计学意义(P<0.05).结论 多孔硅胶管联合猪尾导管应用于胸外科手术引流,较常规硅胶管优势显著,但需依据患者的具体病情予以个体化选择,以期提高引流效果和加速患者康复.Objective To explore the clinical effect of porous silicone tube combined with pigtail catheter in thoracic surgery drainage.Methods A total of 260 patients undergoing thoracoscopic surgery in Department of Thoracic Surgery in the Fifth People’s Hospital of Zhuhai and the 940 hospital of the joint logistic support force of the Chinese people’s liberation army were selected from January 2018 to October 2019. They were divided into experimental group(porous silicone tube combined pigtail catheter and water-sealed bottle)and control group(conventional silicone tube and water-sealed bottle)in 130 cases. Observe the patients’ pain at 12 h, 24 h, 48 h, 72 h after surgery, 24 h after operation and total drainage volume, extubation time, postoperative thoracentesis or closed drainage fluid situation and postoperative hospital stay and other indicators changes.Results A total of 260 patients were included,130 in each group. General data and total postoperative drainage were not significantly different between the two groups(P>0.05). The pain degree 12 h、24 h、48 h、72 h patients in the combined group, the drainage flow rate, extubation time, the number and times of repeated chest penetration after operation, and the postoperative hospitalization time were better than those of the conventional group 24 h after operation(P<0.05).Conclusion Porous silicone tube combined with pigtail catheter is superior to conventional silicone tube in thoracic surgery drainage, but it should be selected individualized according to the patient’s specific condition in order to improve the drainage effect and accelerate the recovery of patients.
