席汉综合征合并Graves病1例并文献复习

摘　　要：目的提高临床医生对垂体功能低下患者甲状腺功能变化的诊疗思路。方法对本院收治的1例席汉综合征患者经治疗多年后出现Graves病的临床资料进行分析,并通过查阅pubmed、万方、维普、知网等国内外多个数据库资料进行回顾分析。结果国外有1例类似报道,国内尚无报道。席汉综合征患者定期复查评估垂体各轴功能状态时,应注意到甲状腺功能可能存在相互转化,也可能存在Graves病可能,应注意及时调整临床治疗方案,并注意甲状腺功能亢进对皮质醇-肾上腺轴功能的影响。结论席汉综合征患者治疗过程中,甲状腺功能可能存在多种变化可能,临床应注意识别并调整治疗方案。Objective To improve clinicians'diagnosis and treatment of hypophysis patients with changes in thyroid function.Methods The clinical data of Graves'disease of a patient with Sheenhan's syndrome admitted to our department after many years of treatment were analyzed.The database data at home and abroad from pubmed,Wanfang,Weipu,and HowNet were reviewed and analyzed.Results There is a similar report abroad,but no domestic report.When the patients with Sheenhan syndrome regularly reexamine and evaluate the functional status of each axis of the pituitary gland,it should be noted that there may be mutual transformation of thyroid function,there may also be Graves'disease.It is necessary to pay attention to the timely adjustment of clinical treatment plan,and to the effect of hyperthyroidism on the function of cortisol adrenal axis.Conclusion During the treatment of patients with Sheenhan syndrome,there may be many changes in thyroid function,and clinical attention should be paid to identifying and adjusting the treatment plan.
