经皮电刺激8周疗法对压力性尿失禁患者盆底肌肉参数及性功能的影响

摘　　要：目的探讨压力性尿失禁患者使用经皮电刺激治疗后盆底肌肉参数变化及其对性功能的影响。方法选取广州市荔湾区人民医院2015年11月至2018年10月收治的压力性尿失禁患者80例纳入临床研究,记为SUI组,同期选取80例正常人进行对照,记为正常对照组。使用女性性功能指数调查量表(FSFI)测定研究对象性生活水平,对比两组盆底肌肉表面肌电信号活力值、峰值、功值及右侧盆底肌休息值;同时按照尿失禁评分将SUI组分为SUI1组、SUI2组、SUI3组,对比3组盆底肌肉表面肌电信号活力值、峰值、功值及右侧盆底肌休息值。随后使用经皮电刺激治疗SUI组8周,对比治疗后SUI组各指标变化情况,随访治疗后3月、6月及12月SUI组性生活质量。结果正常对照组与SUI组活力值、功值、峰值、右侧休息值差异有统计学意义(P<0.05);正常对照组FSFI评分显著高于SUI组(P<0.05)。SUI1组治疗后盆底肌表面肌电信号各参数与正常对照组对比,差异无统计学意义(均P>0.05);SUI2组、SUI3组活力值、功值、峰值与正常对照组对比,差异有统计学意义(均P<0.05);治疗后,SUI组肌力明显改善。FSFI评分与受检者盆底肌表面肌电信号、肌力水平呈正相关性,且与活力值、功值、峰值及肌力水平相关性明显(均P<0.05)。结论SUI患者经皮电刺激治疗具有较好的质量效果,患者盆底肌肉参数与性功能呈正相关性。Objective To investigate the changes of pelvic floor muscle parameters and the effect on sexual function in patients with stress urinary incontinence after percutaneous electrical stimulation.Methods 80 patients with stress urinary incontinence treated at Liwan District People's Hospital from November,2015 to October,2018 were selected for the clinical study,and set as an SUI group.And 80 normal people during the same period were selected as a normal control group.FSFI was used to measure their sexual life levels.The activity values,peak values,work values,and rest values of the right pelvic floor muscle surface EMG signal of the two groups were compared.The SUI group was divided into an SUI1 group,an SUI2 group,and an SUI3 group according to their urinary incontinence scores.The activity values,peak values,work values,and rest values of right pelvic floor muscle surface EMG signal of the three groups were compared.After 8 weeks'treatment,the quality of sexual life in the SUI group was followed up for 3,6,and 12 months.Results There were statistical differences in activity value,work value,peak value,and rest value on the right side between the normal control group and the SUI group(all P<0.05).The FSFI score of the normal control group was significantly higher than that of the SUI group(P<0.05).After the treatment,there were no statistical differences in the parameters of pelvic floor surface EMG signal between the SUI1 group and the normal control group(all P>0.05),but were in the activity value,work value,and peak value between the SUI2 group and the SUI3 group on one hand and the normal control group on the other hand(all P<0.05).After the treatment,the muscle strength of the SUI group was significantly improved.The FSFI score was positively correlated with the surface EMG signal and muscle strength level of pelvic floor muscles,and was significantly correlated with the activity value,work value,peak value,and muscle strength level(all P<0.05).Conclusion The quality of patients with stress urinary incontinenc
