自然分娩产妇分娩减痛效果的影响因素分析与护理对策

摘　　要：目的探究自然分娩产妇分娩减痛效果的影响因素与护理对策。方法选取2018年1月至2019年6月期间在本院自然分娩的280例产妇作为研究对象,均给予无创分娩镇痛仪进行阵痛,并通过视觉模拟(VAS)评分法对产妇无创阵痛效果评价,VAS评分>4分为阵痛效果不佳组87例,VAS评分<4分为阵痛效果良好组193例。采用单因素以及多因素logistic回归分析,明确其镇痛效果的影响因素,并实施相应的护理干预。结果单因素分析显示,产次、妊娠期合并症、产程疲劳是分娩镇痛效果的影响因素,差异均有统计学意义(均P<0.05);多因素分析结果显示,妊娠期合并症(OR=11.358,95%CI=2.824-45.677)、产程疲劳(OR=11.245,95%CI=1.007-1.235)与分娩镇痛效果存在正相关,产次(OR=0.089,95%CI=0.028-0.285)与分娩镇痛效果存在负相关。结论产次、妊娠合并症以及产程疲劳是自然分娩产妇减痛效果的影响因素;因此,在自然分娩中,根据患者情况制定相应的护理措施,提高镇痛效果。Objective To explore the factors affecting pain reducing effect in puerperae taking natural delivery and the nursing measures.Methods 280 parturients who gave birth naturally at our hospital from January,2018 to June,2019 were treated with noninvasive delivery analgesia instrument.The effect of noninvasive paroxysmal pain was evaluated by visual simulation(VAS)score.Those with VAS score>4 were set as a poor paroxysmal pain group(n=87),and those with VAS score<4 a paroxysmal pain group(n=193).Single factor and multivariate logistic regression analysis were used to clarify the influencing factors of analgesic effect.Corresponding nursing interventions were carried out.Results The univariate results showed that labor times,pregnancy complications,and labor fatigue were factors influencing labor analgesia(all P<0.05).Multivariate analysis showed that pregnancy complications(OR=11.358,95%CI=2.824-45.677)and labor fatigue(OR=11.245,95%CI=1.007-1.235)were positively correlated with labor analgesia,and labor times(OR=0.089,95%CI=0.028-0.285)negatively.Conclusion Labor times,pregnancy complications,and labor fatigue are factors influencing pain reduction effect in parturients taking natural delivery.Therefore,in natural delivery,according to their situations,corresponding nursing measures should by formulated to improve analgesia effect.
