摘　　要：目的:探讨传统与改良Skoog法两种手术方法治疗中重度乳头内陷矫正效果及远期随访情况。方法:回顾分析中国科技大学第一附属医院安徽省立医院整形外科2008年1月-2018年1月中重度乳头内陷的47例患者分为传统Skoog法组与改良Skoog法组,分别采用传统Skoog法,改良Skoog法(四瓣法辅以乳管内插入留置针芯、注入亚甲蓝及四瓣与乳管周围组织荷包缝合)。比较两组患者术后乳头直径、乳头高度变化及随访2个月及24个月的情况。结果:传统Skoog法组术后乳头高度为(8.46±1.99)mm,乳头直径为(9.26±1.47)mm;改良Skoog法组术后乳头高度为(11.65±1.27)mm,乳头直径为(9.43±1.96)mm,两组间乳头高度比较差异有统计学意义(t=6.330,P<0.05),乳头直径比较差异无统计学意义(t=8.250,P>0.05)。术后2个月均未出现复发,连续随访24个月,传统Skoog法组4例复发,改良Skoog法组1例,两组患者复发情况比较差异有统计学意义(χ2=8.260,P<0.05)。结论:采用改良Skoog法治疗中重度乳头内陷可获得较满意乳头高度及远期稳定的效果,操作简便易行,复发率低,可有效避免乳管损伤,值得临床推广应用。Objective To explore the effectiveness of traditional and modified Skoog method in treating moderate and severe inverted nipple after the long-term follow-up.Methods 47 patients with moderate to severe inverted nipples in the First Affiliated Hospital of University of Sciences and Technology of China were retrospectively analyzed from January 2008 to January 2018.They were divided into two groups:the traditional Skoog method group and the modified Skoog method group.The postoperative effect and the long time following up of the two groups were compared.Results In the traditional Skoog method group,the postoperative nipple height was(8.46±1.99)mm,and the diameter of the nipple was(9.26±1.47)mm.In the improved Skoog method group,the postoperative nipple height was(10.49±2.77)mm,and the diameter of the nipple was(9.43±1.96)mm.The postoperative nipple height in the improved Skoog method group was signifi cantly higher than that in the traditional Skoog method group(t=6.330,P<0.05),and there was no significant difference in the changes of diameter between the two groups(t=8.250,P>0.05).After 24 months of follow-up,4 patients in the traditional Skoog group and one patient in the improved Skoog group were recurred.The difference of recurrence in two groups was statistically significant(χ2=8.260,P<0.05).Conclusion The improved Skoog method in treatment of patients with moderate and severe inverted nipple is safe and effective.It can achieve satisfi ed nipple height in long-term and is easy to operate and has a low recurrence while effectively avoid the injury of breast duct,which is worthy of clinical promotion and application.
